AAIB Malaysia outlines 5 key Recommendations as Report said Bandar Elmina crash caused by Crew Accidentally deploying Lift Dump Spoiler The final report released by the Air Accident Investigation Bureau (AAIB), under Ministry of Transport Malaysia, found that the primary cause of the accident was the inadvertent extension of the lift dump spoilers, most likely by the pilot who was the second-in-command, during the Before Landing checks.   In the final report released on the Transport Ministry's website today, Jet Valet Sdn Bhd is also advised to improve its compliance with regulatory requirements, enhance organisational oversight, and refine its operational procedures. Breaking News: Aircraft Belonging To Dana Air Skids Off Lagos Airport Runway. Dana Air confirmed a runway excursion incident involving one of its aircraft flying from Abuja to Lagos, with 83 passengers onboard. A runway excursion happens when an aircraft veers off the runway during takeoff or landing. Fortunately, all passengers were safely disembarked without any injuries reported. Dana Air promptly notified the Nigerian Safety Investigation Board (NSIB) and the Nigeria Civil Aviation Authority (NCAA) about the incident. As a precautionary measure, the airline's maintenance team grounded the aircraft pending further investigation into the cause of the incident. Such incidents are taken seriously by aviation authorities and airlines to ensure the safety and security of passengers and crew members. Recently the Air Accident Investigation Bureau under the Ministry of Transport Malaysia released the full investigation report on the air crash that occurred last year in Elmina. I won't repeat the findings here, but the sharing of this report with the public means many will learn from it. The root causes and lessons learned should not only be restricted to the aviation industry. Themes such as functional checklist, system induced human error, and lapse in judgement is also common in the manufacturing industry. Few days ago media was placing Akasa on Zenith when they ordered big batch of Aircraft, now suddenly they have problem of Captains, it’s being placed on Nadir. My Elderly advice & Word of caution for media here. Pls stop casting perception on every instance and please report matter in original form. Let people see further & make own opinions as many small investors, employees, vendors have lost huge money in the Airlines hyped most by media. Pls take this as my very humble request.
