King Faisal Specialist Hospital and Research Center (KFSHRC) has successfully performed 5,000 kidney transplants since the inception of its Transplant Program in 1981, placing it among a select group of leading global health centres that have achieved this milestone. Last year alone, KFSHRC performed 80 pediatric kidney transplants, the highest number conducted by any hospital in a single year. This achievement makes KFSHRC’s Kidney Transplant Program the largest of its kind, surpassing health centres in the United States and Europe. Sickle cell disease (SCD) significantly impacts thousands in the UK, yet treatment options remain limited. In honour of World Sickle Cell Day, we're showcasing our ongoing efforts to empower healthcare professionals with the knowledge and resources they need to better understand and manage sickle cell disease. We recently convened a 7-month digital advisory board with 11 leading UK clinicians, specialising in adult or paediatric care. Their insights proved invaluable help in supporting a successful NICE submission for our client, ultimately securing NHS funding for their medication and offering hope for 4,000 patients. The text discusses a case involving a 32-year-old male patient with Hodgkin's lymphoma who died due to a broken endotracheal tube (ETT) that was aspirated and entered the right bronchus and mediastinum, causing airway obstruction and inability to ventilate. The patient was initially admitted for pneumonia and subsequently transferred to the ICU for intubation. After an hour of intubation, the ventilator alarm sounded, and it was discovered that a part of the tracheal tube had disappeared, leading to cyanosis and unstable hemodynamic parameters. Attempts to extract the tube were unsuccessful, and the patient went into cardiorespiratory arrest, leading to his ultimate demise. The case is a rare incidence of bronchus rupture and mortality as a result of aspirating an ETT, and there are few similar reports in medical literature. The authors highlight the importance of vigilance and experience of medical personnel in the ICU, appropriate IV sedation, and the use of a bite block to prevent such incidents. The text also suggests that in cases of respiratory arrest and failed attempts to retrieve the aspirated tube, reintubation with a smaller diameter tube could be attempted as a last resort, although its success remains unclear. The text emphasizes the importance of meticulous observation in airway management, especially outside the operating room, to avoid risks such as ETT dislocation, esophageal intubation, and unwanted extubation.
