Should governments run health services? The key principles of the NHS are that it is free at the point of use, funded through taxation, and accessible to everyone regardless of their ability to pay. When it was founded and since, we have assumed that the government should manage hospitals and other health services. It is not clear why this has not been challenges. In the late 1991 the Thatcher government introduced GP fundholding: this allowed GPs a budget to pay for care from any qualified provider, which included independent sector providers. The Blair government subsequently expanded the use of the independent sector, for example through its Independent Sector Treatment Centre (ISTC) programme. Now independent sector hospitals carry out about 10% of elective procedures. Despite that, state funding and state provision remain yoked together. There is no intrinsic relationship between the two. We could achieve the principles of the NHS through state funding and entirely independent sector provision. There is a persistent belief that it is obvious that care is cheaper if provided by a state-owned hospital rather than a privately-owned one. Doubtless there is the additional cost of the profit independent sector providers would seek, but this might be more than offset by savings through increased efficiency. State provision of healthcare is intrinsically inefficient because of the need to meet the needs of a plethora of stakeholders. Quality of care, cost-effectiveness, and strategic coordination are all possible through a robust commissioning process. We should not assume that they require state provision.

Quiz Time! I was chatting to a friend (not in healthcare) about the 10-year plan consultation and I found myself trying to describe the organisational structure for NHS primary care in England to a layman 🤔 My suggestion for Wes is that some clarity, if not simplification, of the landscape would be progress for all stakeholders – we shouldn’t assume that it is easy to understand, and that’s just for primary care. As it’s Friday, and to illustrate my point, I’ve created a little quiz.... 🤩 Which ‘entities’ are described below – choose from ICS, ICB, Place, GP Fed, PCN and Practice (definitions are mostly NHSE or Confed)? A - ...collaborative arrangements between organisations responsible for arranging and delivering health and care services and others with a role in improving health and wellbeing. ”Over the past 40 years, many health-care systems that were once publicly owned or financed have moved towards privatising their services, primarily through outsourcing to the private sector. But what has the impact been of privatisation on the quality of care? A key aim of this transition is to improve quality of care through increased market competition along with the benefits of a more flexible and patient-centred private sector. However, concerns have been raised that these reforms could result in worse care, in part because it is easier to reduce costs than increase quality of #health care. ” ”We found that hospitals converting from public to private ownership status tended to make higher profits than public #hospitals that do not convert, primarily through the selective intake of patients and reductions to staff numbers. We also found that aggregate increases in privatisation frequently corresponded with worse health outcomes for patients. Very few studies evaluated this important reform and there are gaps in the literature. However, based on the evidence available, we provide evidence that challenges the justifications for health-care privatisation and concludes that the scientific support for further privatisation of health-care services is weak.” #lifestylemedicine

'The NHS budget is not being spent where it should be—too great a share is being spent in hospitals, too little in the community, and productivity is too low.' This is one of the key findings of Lord Darzi's Independent Investigation of the National Health Service in England, which was published today. It paints a honest and stark picture of the troubled state of the NHS, yet finds hope through the underlying fundamentals being strong despite overall challenges. Lord Darzi doesn't set out to make recommendations regarding next steps - he highlights the lack of capital funding, the work needed to improve outcomes and reduce inequalities through access to timely care. He also highlights the need to improve uptake of targeted screening programmes and a need to shift care into the community, moving from 'diagnose and treat' to 'predict and prevent'. These are noble ambitions but the challenges are huge, made more difficult by changing demographics, growing demand, growing cost pressures and a widening workforce gap. So what next? The work starts now on developing the 10-year health plan, which should be published early next year. This will require an associated funding plan and we may have more clarity around funding as part of the Budget on October 30th. In the meantime, systems should consider how they can drive productivity and get the most 'bang for their buck' as additional funding may not be readily available in the short term. There is an associated call to action for all healthcare providers, professional bodies and suppliers - now is a time for partnership and collaboration to address the reform needed to develop new ways of working and models of care.....and help save a broken NHS. https://lnkd.in/ehZapjse
