2 practicing gp docs discussing endo: Doc 1: I do probably 99% of my own endo as well...retreats, post removals, etc. I might refer 1-2 cases per year out of 150-200. It's a big part of our production in our office and I enjoy the occasional challenging case. I would rather do endo than class 2 composites. Doc 2: Ditto. Other than placing implants, endo is probably the most profitable part of my practice. I actually sometimes feel guilty charging my standard fee, even on molars, when they're a piece of cake. Just not guilty enough to reduce my fee:) I couldn't imagine sending all this easy money out to someone else.

Conversation with a GP who finishes around 1000 endos / year for decades: Me: If any new grads asks 1. how long did it take for you to to be this good at endo? 2. How long after graduation did you start doing molar endo? GP: I was not taught to do molar endo in Europe at all, only anteriors and bicuspids. If someone came in with an infected molar, it was automatic extraction, which always made me sad. It took me many years to get good at endo, but not because endo is so hard. Because we are taught a super complicated, inefficient and risky way of doing endo. We are taught to use many hand files and introduce many large stiff rotary files into the canals. This is very time consuming, risky and destroys the delicate apical anatomy. Many people ask me how come I have so many cases with extreme curvatures, accessory canals, bifurcations and deltas. It’s because I don’t use larger than #20 files, ..... With this technique most molar endo takes 15-20 minutes including access prep to complete obturation. Cheers.

This changes my SRP game- here's why. The love hate relationship I have with SRP's and trying to maintain my 70%- Ultrasonics v. 30% Hand instrumentation rule.
