💊 Frail older adults are at increased risk of medication-related harm, highlighting the importance of medication reviews in this population. 💊 While community pharmacists are increasingly involved in initiating medication reviews, traditional frailty assessment methods are often impractical in this setting. In our recently published study in Drug & Aging, we present a novel frailty screening tool to identify frail older adults. This study demonstrates the potential of using pharmacy dispensing data to screen for frailty in community-dwelling older individuals with polypharmacy. This tool can help inform patient selection for pharmaceutical care interventions, such as pharmacist-led medication reviews, within a multidisciplinary approach. A 2018 study by Watanabe et al. found that the estimated annual cost of prescription drug-related morbidity and mortality due to poor medication adherence—cutting pills in half, skipping doses, or not filling prescriptions—was over $500 billion in 2016. Pharmacists advise on appropriate drug dosing for patients with declining renal function due to aging. Kidney function declines as we age and this has implications for appropriate dosing of many medications. This is just one of the many pharmacokinetic changes in the body due to aging. As many medications are renally excreted, older patients are at higher risk of drug toxicity due to accumulation of the drug in the body. Measuring the glomerular filtration rate (GFR) and serum creatinine give an estimate of how well the kidneys are filtering waste (and drugs) from the body. By considering kidney function, the dosage of renally excreted drugs can be adjusted and the risks of drug reduced. Collaborate with pharmacists for optimal medication management in the elderly.
