Median Arcuate Ligament Syndrome. A difficult diagnosis to make, but must be top of mind for physicians with patients who have vague upper abdominal pain after eating. The median arcuate ligament of the diaphragm compresses the celiac trunk near its root, essentially leading to demand ischemia of foregut structures during meals. Mesenteric doppler studies and/or mesenteric CT Angiography, both done at the end-expiratory phase of breathing, can help elucidate this anatomic abnormality. #RoboticSurgery for this pathology is an ideal approach, given the level of dexterity, visualization, camera stabilization, and fluorescence technology all within a single surgical device.

The Ascyrus Medical Dissection Stent (AMDS) is a very good option in treating acute aortic dissection type A in selected patients. Individual aortic anatomy regarding potential contraindications and proper device implantation are strongly recommended to avoid complete central AMDS collapse. Stretching the felt during performing the anastomosis might be one key of success avoiding aortic stenosis and collapsing . Many thanks to Davide de Manna for sharing the illustrations. In this case we did a root replacement, ascending replacement, AMDS (because of malperfusion) and bypass to RCA.
