Looking at MBS codes for TAVI Implants in Australia, it's pleasing to see the overall uptake in this therapy. Patients at Intermediate and Low surgical risk, however, don't seem to be benefitting as much as expected, with their percentage of the total implants remaining stable since the introduction of their relative codes. The key to diagnosing these patients at the Primary Care level before they become "High Risk" candidates is Auscultation. What an endovascular masterpiece of a case by the legendary, Dr. Leo Fong, ladies and gentlemen! -Smoker -Existing stents -Symptomatic for roughly a month -Off their oral anticoagulant due to a pneumothorax This patient developed a massive arterial LLE occlusion up into the iliacs. Dr. Fong skillfully wired from an up and over approach and recanalized using BOLT7 within 2 passes. After further attempting to work with BOLT7, we knew it was too large and organized of a material. Even after several angioplasty attempts, the material that remained in the proximal iliac remained unchanged. BiVACOR, a clinical-stage medical device company, has announced the successful completion of the first phase of its FDA Early Feasibility Study (EFS) for the BiVACOR Total Artificial Heart (TAH). Between July and November 2024, five patients with severe biventricular heart failure were implanted with the BiVACOR TAH as a bridge to donor heart transplant. Each patient was supported by the TAH for up to a month while awaiting a donor heart. All five patients successfully received heart transplants and were subsequently discharged from the hospital. Following these positive outcomes, the U.S. Food and Drug Administration (FDA) has approved the expansion of the EFS to include an additional 15 patients, bringing the total to 20 participants. The BiVACOR TAH represents a significant advancement in artificial heart technology. Utilizing magnetic levitation (MAGLEV) technology, it features a single moving part—a magnetically suspended dual-sided rotor—that pumps blood to both the pulmonary and systemic circulations. This design aims to minimize blood trauma and eliminate mechanical wear, potentially offering a durable and reliable heart replacement solution. Re-entry catheters can be a really life jacket and save us from complex situations, converting a technical failure into a clinical success. This time we present a CLTI patient with RC5 and a long SFA occlusion. We were not able to cross the lesion from antegrade nor retrograde, and despite multiple maneuvers we failed reentering into the true lumen. We decided to advance a 2.9 Fr Beback catheter (Bentley) from retrograde (P3) and puncturing a balloon inserted from antegrade into the subintimal space, allowing to advance the retrograde guidewire inside the balloon. We continue removing the balloon at the same time we advance the guidewire inside it, this way we achieve a through-and-through with the guidewire externalized from both approaches. Next step, 3' PTA with a 6x240mm NC Jade Balloon (OrbusNeich Medical Company Ltd., B-endo, Be Endovascular) and completed with sirolimus DCB 6x150mm (x2) Selution SLR (Cordis). #CAHU
