Lessons Learned: Tank Overfill Incident This video explains a tank overfill incident and highlights the importance of proper safeguards, procedures, and maintenance. Here’s a simple breakdown of what went wrong: 1. Safeguard Failures: The high-level alarms and indicators failed due to calibration issues and a power shutdown during maintenance. Procedures didn’t guide the team on how to handle the loss of primary level controls. 2. Human Errors: A drain valve was left open during heavy rainfall, allowing spilled gasoline to escape into the ocean. A new high-high alarm was installed without proper calibration, and this mistake wasn’t caught during testing. 3. Response Issues: Emergency spill response equipment wasn’t maintained, delaying cleanup efforts. Key Lessons: Always test and validate safety systems, especially after maintenance or upgrades. Follow detailed procedures for unusual scenarios like power losses. Ensure emergency equipment is ready when needed. This incident is a reminder that even small oversights can lead to big consequences, but by learning from these events, we can improve safety for the future.
