I would reduce the loop and obtain deep PD wire access. This will help better secure the PD wire in place. I will then attempt the double wire technique which works most of the time. If double wire technique fails, i push 5 cm modified stent in the PD and cannulate over the stent. If double wire technique is successful, then i finish the Ercp and place a 5fr x 5cm modifed PD stent. The shorter stent have a very high risk for immediate migration. If I place a cbd stent, I can repeat ERCP in 4 weeks to assess both stents. If i didn’t place a cbd stent, i get a kub in 2 weeks
