Communication can always be improved. Communication gaps exist between hematology-oncology professionals and patients. I bet these consequences are driving worse outcomes: · Increased patient anxiety around therapy due to unknowns · Patient distrust of provider(s) due to lack of clear explanations · Clinical trial participation is low (referenced below for more detail) · Patients hold off on reporting “expected” side effects that can indicate more serious issues (e.g. diarrhea or altered breathing with Keytruda) · Inability for patients to differentiate between immunotherapy, chemotherapy, targeted therapy, etc. – critical info to managing care · Non-adherence and discontinuations are more likely · I’m sure there are more… Clinical could learn strategies from sales/marketing about how to communicate differently with patients in order to better stick these ideas & concepts around treatment. Cancer is a top driver of healthcare spend for employers, often the #1 driver of high-cost claims. Clinical trials should be considered as an opportunity to mitigate these costs. Oncology clinical trials can help patients access cutting-edge care and are pivotal in advancing cancer treatment, while offering a unique opportunity for employers to manage cancer costs. But low enrollment rates for oncology trials hinder these benefits. In Private Health Management’s new whitepaper, “The Importance of Clinical Trials in Cancer Care,” my colleagues, Eva J. Tips for anyone new to clinical research-----Part 2! 1. When working with patients on clinical trials, take the time to explain everything. Often, you’ll be surprised by how well-versed patients are regarding cancer treatment and they’ll appreciate the wealth of information. 2. Encourage patients to ask questions! The freedom to ask questions, builds trust and will take you a long way when working on clinical trials. 3. When you don’t know something, conveying that openly also builds trust. When working with patients, make sure to follow it up with, “but I can find that out for you.” Science isn’t just about knowing everything, it’s also about being cognizant of what you don’t know. 4. Sponsors and CRO’s are your allies, lean on them. 5. Assess the difficulties you may have in implementing a clinical trial early on, so that you can address them proactively. You’ll usually find that a problem that once seemed daunting soon becomes an area of strength and comfortability.

20 mins is the average time for an oncologist consultation… For patients, ongoing discussions about healthcare goals, values, and preferences throughout their cancer journey are crucial. But patients require sufficient time to grasp all the context and challenges, particularly given the highly vital and emotional nature of the subject matter. However, oncologists often lack the time for effective communication with each patient. At times, it's also often necessary to demedicalize the discourse. Shifting focus, sometimes it's essential to change the conversation's tone. That's where integrative oncology can make the difference, focusing not solely on the disease but on the patient in all their dimensions, with a more holistic approach to care.
Conversations between clinicians and patients with cancer about patients’ goals, values, and preferences in the context of their health must happen throughout their care journey, not just at the beginning or end. Yet, these “serious illness communications” are not occurring as they should in routine oncology care. Experts will discuss the importance of these communications and how to implement them into routine care at the Education Session “Let’s Have the Conversation: Scaling Serious Illness Communication in Routine Oncology Care” during the 2024 ASCO Annual Meeting. #ASCO24 “We hope that attendees will apply the broader definition of serious illness communication and integrate these strategies routinely into the care for people with cancer,” said Session Chair Manali Patel, MD, MPH, MS, FASCO, of Stanford School of Medicine. One area that Dr. Patel wants to emphasize is the importance of “demedicalizing” these communications. For example, she noted that, in the current care paradigm, “we have relied on clinicians to initiate these conversations when a wealth of data show that perhaps this should not be the case. Presenter Christopher Manz, MD, MSHP, of Harvard Medical School will focus on the common barriers to these communications and strategies for overcoming them. Barriers include reimbursement challenges, financial misalignment of practices, time needed to integrate these conversations into care, and training required for clinicians and other staff. I do use PRO-CTCAE and all of our patients get a paper version of the form to fill out. I find it a great way to start the conversation, and I agree it isn’t perfect. I need to clarify the scores every time (someone reports pain; it might be chronic back pain not related to the cancer therapy. Swallowing difficulties might be related to dryness, lymphedema, fibrosis, or surgical changes, etc). I never use it on a weekly basis. I think the patient’s perspective on their outcomes and day-to-day life is very important. While the PRO-CTCAE tool is imperfect, I am struggling to see clearly superior tools out there. Hopefully this is a starting point for important research.
