Ophthopedia Update:Learning new surgical techniques: are we doing it well?: Shunmugaranjan et al report that surgical experience is strongly associated with better outcomes after endothelial transplantation.1 The authors used prospective national registry data from over 11 000 endothelial keratoplasties, including Descemet stripping (automated) endothelial keratoplasty (DSEK/DSAEK) or Descemet membrane endothelial keratoplasty (DMEK) to evaluate the impact of surgeon experience. The authors provide compelling evidence that surgical expertise is strongly associated graft survival at 2 years, primary graft failure and need for additional interventions for both DSAEK and DMEK. Visual acuity was better among experienced surgeons post DSEK/DSAEK. In medicine, learning curves were used in the 1980s to describe the uptake of new surgical skills for minimally invasive surgery.2 The ‘learning curve’ describes the fact that a learner’s performance and skills improve over time when the task is repeated. Category of debate --> what do you do with a type A intramural hematoma based on what parameters? 🦭 This retrospective study of 98 patients evaluates the relation between ascending aortic length and outcomes in patients with acute type A intramural hematoma managed medically. It reported that patients with an ascending aortic length of 11 cm or more had a significantly higher risk of aortic progression, surgical intervention, and ulcer-like projections compared to those with shorter lengths. Ascending aortic length could help identify high-risk patients who might benefit from early surgical intervention, suggesting its role in risk assessment and treatment planning. I’m thrilled to share the publication of our latest article titled “Beyond Blood Transfusions: Exploring Iron Chelation Therapies in Transfusion-Dependent Beta-Thalassemia” in Annals of Medicine and Surgery. This review highlights key insights into the efficacy and safety of deferiprone, an oral iron chelator, in managing chronic iron overload among paediatric patients with transfusion-dependent conditions like beta-thalassemia: Significant Findings: Deferiprone demonstrated comparable efficacy to existing therapies, such as deferasirox, with a favourable safety profile. Clinical Benefits: Early initiation of deferiprone therapy significantly reduced iron overload symptoms, without severe adverse effects, offering a promising treatment option for paediatric patients. I'm excited to share the latest research on Aquablation therapy, recently featured in The Gold Journal Urology. This study, ‘Safety and Efficacy of Same Day Discharge for Men Undergoing Robotic-Assisted Aquablation Prostate Surgery in an Ambulatory Surgery Center Setting’ not only underscores the feasibility of performing #Aquablation as a day case procedure, but also the transformative impact Aquablation therapy is having on patients, surgeons, and healthcare systems alike. For patients, the ability to achieve same-day discharge (98% of cases) with minimal postoperative pain and a preservation of sexual function thanks to meticulous focal cautery during the procedure. This research reaffirms our commitment to advancing urological care and improving outcomes for all stakeholders involved. Researchers at University Hospital in Oslo wondered how the introduction of crosslinking (CXL) impacted corneal transplant surgeries. It has been fifteen years since CXL came to in Norway; CXL is fully established and well-known among eye care professionals. Doctors at University Hospital perform corneal transplants in a region of Norway with more than half of the country’s population. For those reasons, researchers believed they could gather a fairly accurate picture of the state of corneal transplants among the Norwegian keratoconus community... In the U.S., as well as Norway, corneal crosslinking and improved contact lens technology have resulted in fewer individuals with keratoconus needing a transplant. This trend should continue, and there will eventually be two groups of individuals with keratoconus: older patients with corneal grafts that will need to be monitored and who may need occasional surgery to replace failed grafts, and those patients who came of age after CXL where transplants are rarely recommended.
