Angalia nimevua mpaka mask ili uone huwa Surgeons hatufichi bajia wala kaimati mdomoni mchana wa Ramadhan 😱🤭 Anyway, as we observe the last 10 days of the holy month of Ramadhan, I reflect upon the valuable lessons fasting has taught me, especially in my role as a Surgeon. 10 Things I Have Learnt from Ramadhan as a Surgeon 🧕🏽🥼 1️⃣ Discipline & Dedication Fasting requires steadfast commitment, mirroring the dedication needed in surgical precision. 2️⃣ Empathy Experiencing hunger and thirst firsthand reminds me of my patients’ discomfort, fostering deeper empathy. 3️⃣ Patience The long hours without food or water teach the virtue of patience, a crucial quality in surgery and life. 4️⃣ Resilience Despite the physical challenges, the human body adapts and perseveres—much like the human spirit during challenging surgeries. 5️⃣ Hydration Importance Realizing the critical role of hydration, not just in fasting but also in post-operative care. 6️⃣ Self-awareness Listening to my body’s cues, understanding its limits and respecting its needs. 7️⃣ Mindfulness Being present in the moment, especially during surgical procedures, is key to ensuring optimal patient outcomes. 8️⃣ Community Support The unity and support found in the community during Ramadhan reflect the importance of teamwork in the surgical field. 9️⃣ Nutritional Balance Appreciating the importance of a balanced diet for overall health, aiding in better surgical recovery. 🔟 Gratitude Recognizing and being thankful for the blessings, opportunities and challenges in both fasting and surgical practice. Working within the complexity of US healthcare without adding to its complexity. Communication and collaboration can help reduce the burden and cost of care while optimizing outcomes. Default mode of MSK care should be through PT, with summary and escalation of additional services being based on clinical efficacy, availability of care and resource allocation. "Collaboration between surgeons and rehabilitation professionals is vital for enhancing outcomes for patients with musculoskeletal disorders. Surgeons should ensure early delivery of high-quality, guideline-appropriate care, while rehabilitation professionals are responsible for providing and documenting high-value care to improve decision-making. Criteria for surgery should include a standardized assessment of prior therapies. Enhanced collaboration will lead to better patient outcomes." Would love thoughts on this matter.

Imbalances in medical research are an unfortunate reality. Many groups continue to be under-represented in research, including women, marginalised communities, people of colour, and the entire global South! Because of this, treatment protocols do not properly account for their needs, resulting in sub-optimal clinical care. One example of this is appendicitis, where there is a choice between open surgery and less invasive surgery. Less invasive operations allow patients to recover faster and with a lower probability of complications – improving their quality of life. Unfortunately, in low- and middle-income countries, like South Africa, it can be difficult for doctors to decide between the two operations because the decision is usually based on CT scans or ultrasounds, which are not always available. In these situations doctors often default to open surgery. What this means is that patients end up with a lower quality of life than their counterparts in high-income countries, who would have had the less invasive operation. Healthcare is unequal simply by accident of birth. Michael Brombacher has just published a study investigating how blood tests can inform when less invasive surgery for appendicitis is more appropriate than open surgery. Studies like these allow for better decision-making in settings where expensive imaging is not available, and are important for the advancement of healthcare in these contexts. I believe the day a physician stops practicing his/her craft, is the day he/she begins to start losing the magic of what it means to deliver healthcare. Although I am a CEO of healthcare companies (Pazona MD Urology and VirtuCare), I am first and foremost a urological surgeon. Yesterday, I pushed outside of my comfort zone and learned a new surgery (robotic aquablation for BPH with the PROCEPT BioRobotics system). Why? Because I believe in offering better solutions for my patients. If you are not changing your practice as a physician, then you should either leave clinical medicine or start sending out referrals to more up-to-date colleagues. There are plenty of surgical procedures that I no longer perform because I believe another surgeon has a better tool in their proverbial tool belt for my patients. I also believe there's also a risk to completely leaving clinical medicine and turning 100% healthcare executive. You slowly forget what it was like to deliver healthcare. Your edge goes from razor sharp to slightly dull. You're no longer current with the challenges in the trenches of patient care. This is not meant to criticize any physician executives. You do what's best for you. We need more physicians in the administrative side of healthcare. Rather, I'd like to challenge those of you who are still in the trenches with me, to step outside your comfort zone. Stop performing out-of-date surgeries if there's a better solution for your patients. Use your clinical expertise to start a new practice that shapes a better patient experience and alleviates your personal pain points of being a physician. If you pursue a MBA and healthcare executive position, keep 1 or 2 days a month of clinical medicine so you don't lose your mojo.
