Metabolic adaptations in #energyexpenditure exist even following bariatric surgery. Why it matters: There may be a homeostatic response to weight loss stimulates weight regain in order to maintain or restore an individual's energetic setpoint. A study by Tam et al 2016 found metabolic adaptation in 24 hr energy expenditure and sleep of -190 to -230 kcal/day as early as 8 wks after surgery. Weight loss drugs have gained popularity in recent years. However, is medication the most effective method for slimming down? New research suggests that bariatric surgery—also known as metabolic and bariatric surgery or weight loss surgery—remains the gold standard for weight loss and managing weight-related health markers. Studies presented on June 11 at the American Society for Metabolic & Bariatric Surgery (ASMBS) 2024 Annual Meeting highlighted three major findings: 1. Bariatric surgery is more effective both in the short and long term compared to medications or lifestyle interventions. 2. Bariatric surgery significantly slows or stops the progression of prediabetes to type 2 diabetes. 3. Bariatric surgery is generally more cost-effective than weight loss medications. Is bariatric surgery still a safe and effective option in treating obesity? Our Wondr Health Chief Medical Officer, Tim Church, MD, MPH, PhD was interviewed by Yahoo to dive into misconceptions surrounding obesity and bariatric surgery. "While 2 in 5 adults in the U.S. have taken a GLP-1 drug like Ozempic or Mounjaro for the purpose of weight loss, bariatric surgery continues to be “underutilized,” experts say. The ASMBS reports that in 2022, there were nearly 280,000 bariatric procedures performed in the U.S., which means that only 1% of people eligible for the surgery actually had it. Dr. Tim Church, an obesity researcher and chief medical officer of Wondr Health, tells Yahoo Life that these findings may alleviate the apprehension that many people have toward bariatric surgery. “Surgery is underutilized by patients due to fears about cost, safety, permanency and stigma. Mid-portion Achilles tendon pain is not always related to Achilles tendinopathy since pain related to the plantaris tendon alone was found in almost every fifth patient, as highlighted by this case series. · Clinical examination, ultrasound (US), and colour Doppler (CD) were used for diagnosis. Key Findings: -In 19 out of 105 tendons (18%), plantaris tendinopathy alone was found, with a normal Achilles tendon. -These cases showed distinct tenderness on the medial side but no thickening of the Achilles tendon. -US examination and surgery confirmed the diagnosis. Implications: -Midportion Achilles tendon pain is not always related to Achilles tendinopathy. -The plantaris tendon alone can be a source of pain in the Achilles region. - Proper examination is crucial to identify the correct pain source and establish an accurate diagnosis. https://lnkd.in/gpmkrMaF

There is a growing concern regarding the indiscriminate prescription of GLP-1 RAs to overweight and obese patients worldwide. Emerging evidence suggests limited weight loss in obese individuals and a significant risk of weight regain upon medication cessation. Studies indicate that combining GLP-1 RAs with bariatric surgery may be more cost-effective in the management of obesity.

Understanding Tendon Pain: • The exact source of pain in tendons remains unknown; it’s not due to imaging-detected pathology or vascular markers. • Pain originates from the surrounding area, not from sensory nerves deep within the tendon. Tendon Healing Myths: • Degenerative tendons can’t be healed by surgery, shockwave therapy, injections, or even exercise. • Improvement in symptoms is possible despite no structural changes, indicating the focus should be on functional rehabilitation. Tendon Loading and Recovery: • Tendons struggle more with repetitive stretch-shorten cycles than with heavy, slow resistance exercises. • Combining compressive and tensile loads causes more damage than either load alone. • Intense tendon loading results in collagen degradation for up to 36 hours, necessitating adequate recovery time. Structural Insights and Tendon Ruptures • Pathological tendons have more good tissue than normal tendons, so focus on strengthening the healthy parts. • Most Achilles tendon ruptures are asymptomatic and occur in tendons with underlying pathology, making pain potentially protective against ruptures.
