It's great to see the new NELA report published. The summary shows some positives and some things that should make us pause. My main take home from this graphic: EmLap numbers are high - 27K per year (about 0.04% of the population). This group uses lots of resource (long LOS, critical care admission, complications, etc). This means it has disproportionate impact on services for its numbers. The report flags two key concerns. We are no longer seeing improvements in mortality or length of stay. Have we achieved the maximum yield of doing the same things but faster and with more senior surgeons & anaesthetists in theatre? What are the novel pathway/interventions we can implement to move the needle? How do we prevent complications or minimise their impact in new ways. Dear all, I am glad to share the first protocol for REBOA use in a medicalized pre-hospital system (as we use in France 🇨🇵). To realize this protocol, we used a DELPHI method with 18 experts from more than 7 countries 🫡, in collaboration with #EVTM society. Full article will be published in Journal of Special Operation Medicine fall 2024 edition. The main point concerning the use of REBOA lies in the indication: it should not be given too early to patients who do not need it (at the risk of increasing morbidity and mortality!) and should not be given too late to patients who are no longer savable. Keeping in mind that REBOA is only a temporary solution before damage control surgery. Time to try REBOA in a medicalized pre-hospital emergency departement ?

We asked Chat GPT to anser orthopaedic trauma related questions for three different target groups: patient, nonorthopaedic medical doctor and expert orthopaedic surgeon. Examples of questions were: 1.     What is the recommended treatment of a femoral shaft fracture in an adult? 2.     Which are the indications for radial head replacement after trauma to the elbow? 3.     What is the reoperation rate after undisplaced femoral neck fractures in elderly patients? Full list as supplement for online paper. ChatGPT was asked to answer in an umber of sentences in relation to target group, more detailed and longer answers to experts. The study results indicate that ChatGPT can yield valuable and overall correct responses in the context of orthopaedic trauma surgery across different target groups. A recent qualitative study published in the Annals of Surgery explores the psychosocial experiences of older adults undergoing major elective surgery, shedding light on their unique vulnerabilities. This study, involving 25 patients aged 65-82 and 11 caregivers, identifies significant emotional and social challenges before and after surgery, which critically influence recovery outcomes. The findings stress the need for the healthcare system to provide more robust emotional support to address these challenges effectively, which is often overlooked in current geriatric surgical care practices.
