Despite thorough thought, consultation, and risk management, outcomes are uncertain. Decisions can go wrong, and sometimes, there's no possibility for course correction. 'Get-there-itis' is a potentially critical issue in aviation where pilots prioritize reaching the destination over safety. Similarly, in organizations, continuation bias occurs when sticking to the original plan despite changing conditions. The determination to adhere to the initial course can lead to overlooking necessary changes. #leadership

Are there lessons for business in this? You bet! These organizations are world class because they apply the same discipline that it takes to do this safely to organizational processes: leader standard work, KPIs, training, communications, clear chain of command, and others. https://lnkd.in/duf66SVb

Automotive sector is addressing some of the issues in this case with the new revision of APQP and Control plan. 1. Make leadership accountable for safety and quality of the product and manufacturing process. 2. Analyze and address the risk of the project. 3. Identify and control high risk suppliers. 4. Plan and have enough capacity internally and through the entire supply chain. 5. Document rework and repair and keep traceability records. 6. Maintain a separate control plan for rework performed in an alternate process. 7. Reenter reworked product at the point it was rejected for reinspection. 8. Keep tight control on safety and pass through characteristics. 9. Review lessons learned and implement corrective and preventive actions, specially systemic root causes like lack of safety and quality culture. 10. Do a read across for other products and processes with potential for similar risks. I probably missed some points, but there are lessons learned for all sectors in this case. Ricardo.
