While it's true that we see a plethora of hospitals lining the streets, a closer look reveals a challenging reality. Many of these facilities are operated by non-medical professionals or staffed by practitioners who may not hold an MBBS degree. Consequently, the majority of these hospitals can handle basic treatments—like fevers, dehydration, or minor surgeries—but when it comes to life-threatening conditions such as heart attacks or major accidents, patients often have no choice but to rush to larger cities. The distance can be daunting, and the cost of travel adds an extra layer of stress to an already critical situation. Government initiatives like PMJAY, RSBY, and Swasthya Sathi have certainly enabled the establishment of hospitals in remote areas. However, there’s a disparity in funding—hospitals in smaller locations often receive less financial support than those in metropolitan areas. It's ironic! The government needs to understand that providing tertiary care in remote settings requires 20-30% more investment due to the challenges of arranging specialized manpower and resources. It's time we rethink our approach to healthcare equity. This is an interesting reflection on #ruralhealth in NZ from Pragati Gautama. When I was CD of a small rural hospital a large proportion of my time was taken up in recruitment and rostering and it just wears you down. Rural deserves better. Assuming that #telehealth in these settings will alleviate the problem is flawed on many levels. Telehealth is an augmentation tool, not a replacement tool and staff still have to provide a service. For every patient that a TH doctor is seeing online they are not seeing one in person. Top of scope and augmented practice conditions are key in these sorts of Telehealth solutions. Absolutely a nationwide network of emergency doctors trained in TH could augment services (look to Australia or Ontario for great examples) but ad hoc locum cover without adequate infrastructure is just robbing Peter to pay Paul and not solving the bigger picture. The NZ Telehealth - Forum and Resource Centre have put forward a ‘Patiet Anywhere Clinician Elsewhere’ model formulated on international models and engaging broadly with the sector…. It is time to have serious conversations as to the future of Telehealth in secondary/tertiary care before we move into further ad hoc models. And let’s be clear: Telehealth is not a workforce replacement tool.

Here are four key initiatives to elevate healthcare delivery in remote areas: 1. **Empowering Through Health Education**: Recognizing the importance of health education, we're prioritizing community empowerment through informative sessions on preventive care, hygiene practices, and disease management. By equipping individuals with knowledge and resources, we empower them to take charge of their health and well-being. 2. **Establishing Subcenters for Comprehensive Testing**: To expand our diagnostic capabilities, we're working to establish subcenters equipped with essential testing and radiography facilities. This includes conducting tests such as CBC, thyroid screening, and imaging studies like X-ray and ultrasound, which cannot be performed in our mobile medical unit. By bringing these services closer to communities, we aim to enhance early detection and timely intervention for various health conditions. 3. **Screening for Psychiatric Illness**: Recognizing the significant burden of psychiatric illness, we're implementing proactive screening programs to assess and mitigate mental health challenges. Through targeted screenings and assessments, we aim to identify individuals at risk of psychiatric disorders and provide timely interventions and support services. By addressing mental health needs, we strive to promote holistic well-being and resilience within our communities. 4. **Increasing Regular Follow-up for Chronic Diseases**: Acknowledging the importance of continuity of care, we're enhancing our efforts to provide regular follow-up and monitoring for chronic diseases such as hypertension and diabetes. Through personalized care plans and ongoing support, we aim to improve disease management and prevent complications, ensuring better health outcomes for individuals living with chronic conditions. Together, through a combination of health education, expanded diagnostic services, proactive mental health screenings, and increased follow-up for chronic diseases, we're working to enhance healthcare accessibility and outcomes in remote areas. It’s been estimated that England will need 23,000 to 39,000 more hospital beds by 2030/31 – but there’s only so much room in our hospitals. Caring for patients in their own homes could be one answer, but despite significant funding, there’s still less uptake of remote monitoring technology in virtual wards (also known as “Hospitals at Home”) caring for people with frailty. Our study explored the challenges of implementing remote monitoring in frailty virtual wards, based on the views and experiences of those involved. Here’s what we found: (1) Uncertainty about the benefits: Many healthcare professionals were unsure if remote monitoring was suitable for frail patients. (2) Concerns about changes to patient and carer roles: Remote monitoring would require patients and carers to take on more responsibility in managing health, which could be overwhelming. (3) Lack of confidence in technology providers: The right tools and technology weren’t always available, and products needed to be improved to give frail patients and virtual wards better support. (4) Balancing standardisation with flexibility: Virtual wards varied widely in how they implemented remote monitoring, making it difficult to assess and compare their effectiveness.
