Exploring Surgical Solutions for Fluid Buildup Treating fluid buildup in the abdomen in abdominal cancer, known as malignant ascites, depends on the stage of the cancer. For curative situations like stage 3 ovarian cancer, surgery is often a crucial part of the treatment. Cytoreductive surgery and HIPEC are commonly performed to address this condition. In non-curative cases, palliative chemotherapy is administered along with regular drainage of accumulated fluid guided by sonography. But with 35% toxicity rates in the IO arm - with potential delays in surgery + long-term toxicities in survivors- and high financial barriers to access, we must rationalise which patients actually need it. Identifying those patients most likely to benefit & those most likely to achieve a pCR would streamline our clinical practice significantly. Do we really need Surgical Robot for Minimally invasive / transoral surgeries?? Answer is NO…. A good alternative for surgical robot in resource constrained setting is OLYMPUS ENDOEYE FLEX system, which is 1% of surgical robot cost. We used OLYMPUS ENDOEYE FLEX system for resection of SUPRAGLOTTIC cancer with help of Laparoscopic instruments and harmonic energy source in 72 yr old man. Patient discharged to home next day. Final HPR shows T1NO with negative margins (Stage -1). Swallowing rehabilitation done regularly under supervision and feeding tube removed after 2wks. Scopy after 1month shows excellent speech and swallowing without any morbidity. Always remember…. 1. Think out of the box in Resource constrained settings 2. Utilize whichever resource is available to you in best possible way. 3. Surgical robot is luxury, but remember which was evolved from laparoscopic/endoscopic concept only. 4. Age is just number…. If performance status is good, go for curative intent treatment whenever feasible. This surgery also called as “Trans-Oral Ultrasonic Surgery” (TOUSS).
