Cribriform plate was fractured at multiple sites. Anterior cranial fossa floor repair using Fascia LATA + fat+ pericranium graft was done. Unlike all other cases of cranial trauma, uniquely these cases require skill of skull base surgeon for basal approach, plastic surgeon or Maxillofacial surgeon for reconstruction. They most commonly occur in males in the third decade of life, and are associated with high morbidity and mortality. They may occur following either blunt trauma (90% of cases) or penetrating trauma (10% of cases). 📌Most TIAs are false aneurysms, precluding direct microsurgical repair or endovascular obliteration. D-wave recording is particularly useful in #neurosurgical interventions such as spinal tumors, where it is important to assess the function of the #corticospinal tract cranial and caudal medullary fibers of a #spinal lesion. Special #electrode catheters with several contacts are placed epidurally for signal conduction. In addition to #monitoring the function of motor descending nerve pathways, D-wave recording enables an assessment of the number of functioning, fast-conducting fibers in the corticospinal tract. D-wave signals are less influenced by #anesthetics, heart rate, blood pressure and body temperature compared to #MEP signals, which is an added value in #intraoperative signal interpretation compared to MEP recording. With #AVALANCHE® PLUS, the configuration of the up to 3-channel D-wave lead is done visually via drag & drop.
