5 Red Flags of a Bad Clinical Evaluation Consultant 1. They say they know it all. Seasoned veterans like to make sure what they say is correct and do not jump to an answer right away. 2. They say you need to do everything and right away. They do not provide strategic plans to tackle issues with the scope and budget in mind. 3. They do not respect your expertise. They trample your knowledge and experience and dictate their way. 4. They only try to cover their ass. Instead of working WITH you, they work against you to avoid liability. 5. They show no interest of following up after the work is done. They do not back up their work by following up to ensure the work done was satisfactory.

Criminal medical mismanagement I'm not the brightest bulb in the house. Every halting of my functions is a sure way to cause unnecessary degeneration. It's like I am the only one who has a clue. ALS patients have already arrived with diminished functions and abilities. Unfortunately, very few Healthcare providers and professionals appear to know much about ALS, and instead of helping, they help their residents to an earlier grave than already prognosticated. Hey, but I have the drive and determination to live and enjoy life as I imagine most of you do. I'll begin labeling those who have mismanaged my care under the camouflaged guise of being careful. The way I see it, I have been medically kidnapped against my expressed will or any indication of the need for intervention. I have been limited in much of what practical facilities would provide. I'm still waiting for an answer to what the facility actually provides. Most of you would quickly say, not much. Smile. In some cases, you would be correct. But it is getting better. I now have to prove that I can safely eat what I have been eating for over six months. I'll put it all in a book to help others not to tolerate the same mistakes as long as I have. Certainly, to determine whether a facility can adequately provide for your loved ones needing care. Things are getting better. I'll update you on the progress, positive energy, and cultural improvements. I'm not a hostage, and people should be held accountable when denying patients their rights. Or say, we won't feed you orally, but someone else can. Remember Yoda, do or don't do? I have made the best of many don't do or don't do well… yet. A better day after a while. ALS - Angry Loud Simon. Thanks, Yvette.

📝 Clinical Record-Keeping in Catastrophic Injury Management Clinical record-keeping is a foundation of safe and effective Case Management. information captured in these records can significantly impact the recovery journey and future care of the patient. 📎 Continuity of Care: With precise and comprehensive records, healthcare professionals can ensure seamless transitions between different stages of treatment and work with other professionals. This continuity is vital for tracking progress and adjusting care plans as needed. 📎 Personalised Treatment Plans: Detailed records provide insights into each patient's unique needs, based on their personal goals, and enabling tailored interventions that enhance recovery and quality of life. 📎 Creating a Comprehensive Narrative: Our records don't just list symptoms and treatments; they weave together a story of the patient's experiences and recovery. This narrative is critical for legal teams as it provides a clear, chronological picture of the impact of the injury on the patient’s life. Our system of electronic records also means information is searchable and quickly available. 📎 Legal and Ethical Safeguards: Proper documentation protects both patients and health and social care providers. It ensures that all actions taken are in the best interest of the patient and compliant with medical and professional standards and legal requirements. 📎 Research and Development: High-quality records contribute to research and service development as well as informing the development of the wider health and social care sector, helping to advance treatment methods and improve outcomes for future patients both within and beyond case management services. 📎 Documenting Symptom Progression: Detailed entries help illustrate how the patient’s recovery evolves, which is crucial for demonstrating long-term effects and ongoing needs, or demonstrates a return to independence. This can significantly influence the outcomes of litigation proceedings. 📎 Providing Objective Evidence: Records serve as impartial evidence , offering data and professional observations that can be pivotal in legal arguments. 📎 Enhancing Credibility: A comprehensively maintained record underscores the credibility of the health care professionals involved, reinforcing the validity of the evidence given. This can be particularly influential in court , when the records are relied upon by the Case Manager to assist them in their role as a witness of fact. 📎 Supporting Financial Claims: Detailed accounts of treatments, interventions, and ongoing care requirements help build a strong case for financial compensation. They showcase the extensive medical efforts and resources required for the patient’s recovery. #CaseManagement #ClinicalRecords #Care hashtag#LifeChangingInjuries

The science of #clinicaloutcome assessment is of growing interest and has become essential to medical product development. Clinical outcomes management understands the patient's view of illness and treatment and how the patient feels and functions. To understand if, for example, a treatment improves a patient’s pain, ability to perform daily activities or quality of life, measures must be rigorous, whether collected from the patient, clinician, or caregiver’s perspectives. The purpose of our 2-part series is to define and discuss the types of clinical outcomes, their evaluation, and how to improve them to provide #decision_makers a mechanism to assess treatment progress and measure the results of the services they provide. #healthcareleaders #PHI #Healthinsurance #hospitalmanagers #pharmacists #doctors #claimmanagers
