#NHSEngland's recent announcement to expand #VirtualWards is set to target improved patient flow and clinical outcomes. The expanded rollout will focus on managing conditions like frailty, #HeartFailure, and respiratory conditions, such as #COPD. These are critical areas where timely proactive intervention can make a significant difference in patient health and hospital efficiency. However, the focus of this initiative remains on acute short-term services. A crucial enabler for admission avoidance initiatives is supported self-management to empower proactive selfcare. This reduces the burden on care teams as part of a continuum of care focused on early identification and avoidance rather than short-term pre-acute and acute interventions. By integrating digital tools for self-management and long-term monitoring, multi-disciplinary teams can collaborate across settings, capturing and utilizing relevant data to provide high-quality integrated care. Demand growth for public hospital ED services has exceeded population growth for over a decade, and we need to innovate to keep up. In this interesting dive into emerging local and international models, Damien Bruce and Sarah Wenham identify four categories of innovation to address the rising demand and operational challenges faced by EDs nationally: 1. Identification of care needs through online self-checks and telehealth consultations 2. Presentation and triage innovations, including digital platforms and virtual EDs 3. Diagnosis and treatment advancements, such as the use of AI-based diagnostic algorithms and mobile stroke units 4. Cochrane Australia has recently released insights into the implementation and sustainability of Hospital in the Home programs, along with patient perspectives about those programs: https://lnkd.in/gHNWPjvm "It's encouraging to see the research that's being carried out to understand the benefits of Hospital in the Home," says Vitalis Health Co-founder and Clinical Director Voni Leighton. "Vitalis' work shows that a patient-centred HITH service can successfully prevent hospital admissions (and readmissions), shorten hospital stays for patients, deliver a better patient experience and reduce the burden on carers. Following up on this Becker's Healthcare article and Michelle Stansbury’s comments about today’s new (old) buzzword “Access to Care”. Health Data Analytics Institute (HDAI) recently shared national data showing that only about half of patients who are at high risk for mortality, or 30-day readmission, are seen within 14 days of discharge. The reasons the other half are not seen are often related to access issues. In our recent Becker's webinar with Stuart Dobbs, MD and Brenda Campbell from Houston Methodist, we highlighted how AI can play a role in identifying high-risk patients and their social drivers of health while in the hospital to help prioritize their care and make necessary accommodations to ensure they are promptly and appropriately evaluated post-discharge. This will improve outcomes and reduce the burden on the system by reducing unnecessary readmissions and freeing up much-needed inpatient resources. https://lnkd.in/eHQYnZJG #AIinhealthcare #qualityimprovement #healthcare
