What happens if you see something hazardous at work? In far too many cases they are just left. "It's not my problem," is the common refrain. At Height Safety Engineers we implemented a Hazard Observation process. This straight-forward form provides an easy-to-digest report of what happened, what was done about it and what can be done to avoid a repetition. Risks cannot be mitigated if they not first identified.
