Its an clear example of Near Miss. A near miss is an incident that did not result in injury, illness, or damage, but had the potential to do so. It is an event that could have led to harm, but was narrowly avoided. Near misses are also known as: - Close calls - Near hits - Potential accidents - Incident-free events Near misses can be caused by various factors, including: - Human error - Equipment failure - Process or system failures - Environmental factors The significance of near misses lies in their ability to serve as a warning sign, indicating potential hazards or weaknesses in systems, processes, or behaviors. By investigating and learning from near misses, organizations can: - Identify root causes - Implement corrective actions - Prevent future incidents - Enhance safety and risk management practices Near misses offer valuable opportunities for growth and improvement, making them an essential aspect of safety management and risk reduction strategies.
