Today, I was revisiting and re-reading the basics of ECG, and I had Just an important realization. While we often focus heavily on interpreting the ECG itself, we tend to overlook essential aspects like considering patient history and comorbidities, and maintaining proper patient positioning, correct lead placement, understanding the ECG machine basics, ensuring good contact with leads etc. These factors are just as critical for accurate ECG readings. For example, misplacing V1 and V2 (in the 2nd or 3rd intercostal space instead of the 4th) can significantly alter ECG results, leading to false interpretations. It can cause abnormal P waves (like a negative P in V1), mimic incomplete right bundle branch block (IRBBB), or even create false Q waves that resemble a septal MI. Such misplacements can also trigger false alerts for STEMI or Brugada patterns, especially in young, healthy individuals. To truly master ECG interpretation, we must consider all aspects of the process—not just the readings themselves. Proper lead placement and a comprehensive approach are essential to avoid misdiagnoses and ensure accurate results, especially when assessing conditions like pulmonary embolism or ischemia. For accurate ECG readings, consider proper lead placement, clean skin, good electrode contact, patient posture, and minimizing movement. Ensure the ECG machine is calibrated, free from electrical interference, and maintain consistent electrode placement. Factor in the patient’s age, body type, heart rate, and comorbidities for reliable results and accurate interpretation. _________
