Embracing New Challenges: My Journey into Pediatric Neurosurgery When I first began my neurosurgical training, pediatric neurosurgery was not an area I envisioned myself pursuing. It wasn’t a significant part of my exposure during training, and frankly, I didn't see myself delving into it. However, life has a way of challenging us to grow in unexpected directions. Since starting my new position, I’ve encountered pediatric emergencies more frequently. These cases, while daunting, have opened my eyes to the complexity and stakes of pediatric neurosurgery. The challenges are unique, the risks far greater, and the responsibility immense—but that’s precisely what has sparked my newfound interest in this field. Recently, I had the privilege of working alongside a senior colleague on a challenging pediatric case. A young patient presented with hydrocephalus and a tumor requiring urgent intervention. The journey to surgery was fraught with challenges: the parents were understandably hesitant about moving forward, the tumor bled, and we navigated the complexities of a supracerebellar approach. While we achieved significant resection, a small remnant remains, and we await histopathology results to determine the next steps. From an ergonomic standpoint, the procedure was demanding—pediatric cases often require a completely different skill set and approach compared to adult neurosurgery. Fortunately, our new neurosurgery center is evolving, and we are on the verge of integrating intraoperative MRI, which will be an invaluable tool for such intricate surgeries. I am deeply grateful to my colleague for trusting me with this opportunity and for guiding me through the process. This experience has reinforced the value of teamwork, mentorship, and adaptability in our profession. To my fellow neurosurgeons and healthcare professionals: never underestimate the power of stepping out of your comfort zone. It’s where growth happens. Hemispherectomy, or the surgical disconnection of one half of the brain from the other half and brainstem, is a type of epilepsy treatment for pediatric patients. While it is extreme, this procedure can be an integral step towards improving the lives of young patients with severe or drug-resistant epilepsy. Despite the largely positive outcomes hemispherectomy can offer, there remains a significant amount of variation in seizure outcomes for patients post-hemispherectomy. This variability, along with an array of potential side-effects, can lead to significant anxiety for families deciding whether their child should undergo hemispherectomy surgery. The Hemispherectomy Outcome Prediction Scale (HOPS), a tool spearheaded by Aria Fallah, MD, MSc, Pediatric Neurosurgeon and Dr. Alfonsina Q. Davies Endowed Chair in honor of Paul Crandall, M.D., for Epilepsy Research at UCLA, and a multi-institutional team, was developed to better predict seizure freedom for patients undergoing a hemispherectomy. To make this groundbreaking resource more widely accessible, Dr. Fallah and his team created an online calculator that would accurately predict the probability of seizure freedom post-hemispherectomy. STWs cover common and serious diseases in cardiothoracic surgery, pediatrics cardiology, interventional radiology, neurosurgery, and orthopedics. Workflow standardization helps clinicians to easily follow guidelines, ensuring effective treatment protocols. Pedro-Jose Lopez, MD, Division Chief of Pediatric Urology at UH Rainbow Babies & Children’s Hospital, and his colleague Juan Manuel Moldes, MD, a pediatric urologist at the hospital, are on a mission to make UH a leader in compassionate care, starting with the Pediatric Urologic Surgery division. They’re not alone. The idea of building intentional compassion into healthcare delivery is gaining momentum. In fact, compassion in healthcare is fast becoming an important field of scientific study with its own name: compassionomics. Studies show that the human connection that occurs when delivering compassionate healthcare improves patient outcomes and adherence to prescribed treatment. It also is shown to improve healthcare quality and safety, while reducing costs, the performance of unnecessary procedures and provider burnout. Drs. At nine years old, Carson Brooks was diagnosed with epilepsy. As he grew older, his seizures worsened despite taking an escalating number of pills to treat his epilepsy. To address these debilitating seizures, Aria Fallah, MD, MSc, Pediatric Neurosurgeon and Co-Medical Director of the Epilepsy Surgery Program, surgically implanted a device in Carson's brain to deliver responsive neurostimulation (RNS). RNS continuously monitors brain activity, detects oncoming seizures, and delivers a small electrical current to prevent seizures. RNS has offered hope to the third of patients – like Carson – with drug-resistant epilepsy and is a fairly new option for pediatric cases. RNS, along with a second surgery to remove some of the affected area in his brain, have led to a decrease in the intensity of Carson's seizures and an improvement in his quality of life.
