A pregnant person’s risk for complications doesn’t end with delivery—1 in 7 people develop a high blood pressure disorder during or after pregnancy, putting them at risk for stroke or organ damage.   M Health Fairview keeps an eye on postpartum patients with elevated blood pressure after they’ve left the hospital with a six-week home-monitoring program through MyChart. Patients record their blood pressure twice a day, and a team of nurses monitors that data and intervenes when necessary.   One year in, the program has served 367 patients and has led to a 69% reduction in postpartum readmissions due to hypertension. It’s common to go through hormonal, physical, and emotional changes during and after pregnancy. Symptoms of postpartum depression can also start while you’re pregnant, or after you’ve given birth. About 1 in 8 women report experiencing symptoms of PPD and is underdiagnosed either and or left untreated! Do you offer any kind of program for this problem as well?

69% reduction is excellent ! I wish this was incorporated in UT health San Antonio. There are just not enough providers to provide post partum services for the large population that is in need. Post partum appt are far and few, and the population is very sick with multiple co morbidities .

This is exactly what I am looking for here in Houston, Tx. I would venture to include high risk OB such as known chronic HBP, multiples, infertility treatments to name a few. Does anyone know of a program, pilot or established, here in Houston looking for OB RN’s to possibly do home visits as well as patient own testing? There is such a need. Thank you.

Postpartum programs are extremely important. In many cultures, postpartum care is inevitable. It is the foundation of the relationship between the mother and baby.
