Other research groups have examined the reliability of inducing CNV via subretinal PEG injection. In a study aimed at examining the reliability and reproducibility of inducing CNV according to Lyzogubov et al., Fernando-Bueno and colleagues found inconsistency in the ocular changes described in PEG and PBS-injected mice [42]. Specifically, they identified early histological signs of CNV within PEG-injected mice, including degenerative changes in the external retinal layer, disruption of the RPE and choroid with vacuolization, and increased IB4 immunoreactivity. These findings were consistent with reported findings from Lyzogubov et al. However, these findings were heterogeneous and inconsistent within PEG-injected mice, and some of the early signs of CNV were also present in PBS-injected mice [42]. Moreover, no significant VEGF or von Willebrand factor (vWF) activity was identified in PEG or PBS-injected mice on day 14 after injection [42]. This report differs significantly from previously described dose-dependent effects of PEG on CNV and the important absence of CNV within PBS-injected mice (Fig. 1). Endometriosis: Saliva Test Promises Faster Diagnosis Nine out of ten women with endometriosis believe their quality of life would have been significantly better if they had received an early diagnosis. However, the average diagnostic delay for the disease remains between 7 and 10 years. The introduction of the Endotest saliva diagnostic test could be a game-changer. Professor Sofiane Bendifallah, an obstetrician-gynaecologist at the American Hospital of Paris, highlighted this potential transformation during the Pari(s) Santé Femmes congress held from 12-14 June, 2024, in Paris. He emphasised that the new test could revolutionise the lives of the affected women and streamline their care pathways. "Although we have improved our listening and the examinations carried out, many forms of endometriosis remain undiagnosed or poorly diagnosed in imaging, particularly superficial, millimetric, or peritoneal forms," acknowledged Bendifallah. "These forms represent the vast majority of endometriosis cases. This explains the discrepancy between what women report and our imaging results." According to a study by the associations EndoFrance and Endomind, 65% of women with endometriosis had to undergo numerous examinations, including at least three ultrasounds, and a quarter required surgery to obtain a diagnosis The Saliva Test The introduction of the Endotest saliva test marks a revolution. The test analyses salivary microRNAs through high-throughput sequencing. Endometrial samples reveal the presence of microRNAs specific to the disease's pathophysiology, which can serve as biomarkers and can be found in all biological fluids, including blood and saliva. Some of these microRNAs are encapsulated, allowing them to be detected in saliva. Through artificial intelligence, a specific molecular signature of the disease has been identified based on 109 microRNAs. The developed test has sensitivity, specificity, and area under the curve all above 95%, according to a study conducted on 1153 women from 18 centres in France. In December 2023, If the test is negative, "hormonal prescription is no longer logical, given that the suspicion of endometriosis has been eliminated”, Bendifallah noted. “We are entering a new period. These women, who experience chronic pain of another origin, are deserving of attention. It is likely that there are thousands of them, and further investigations will be needed to propose a change in their care."

📃Scientific paper: Effects of resection volume on postoperative micturition symptoms and retreatment after transurethral resection of the prostate Abstract: Purpose Despite advances in technology, such as advent of laser enucleation and minimally invasive surgical therapies, transurethral resection of the prostate (TURP) remains the most widely performed surgical technique for benign prostatic hyperplasia (BPH). We evaluated resection volume (RV)-derived parameters and analyzed the effect of RV on post-TURP outcomes. Methods This observational study used data from patients who underwent TURP at two institutions between January 2011 and December 2021 Data from patients with previous BPH surgical treatment, incomplete data, and underlying disease affecting voiding function were excluded. The collected data included age, prostate-specific antigen, transrectal ultrasound (TRUS)- and uroflowmetry-derived parameters, RV, perioperative laboratory values, perioperative International Prostatic Symptom Score (IPSS), follow-up period, retreatment requirements and interval between the first TURP and retreatment. Results In 268 patients without prior BPH medication, there were no differences in prostate volume (PV), transitional zone volume (TZV), or RV according to IPSS. A total of 60 patients started retreatment, including medical or surgical treatment, within the follow-up period. There was a significant difference in RV/PV between the groups without and with retreatment respectively (0.56 and 0.37; p = 0.008). Neurovascular-Nugget 23 Reading of this manuscript may reflect 🎹 Key Findings: 🎹 👆Introduction: - IH frequently complicates CVT, affecting around 40% of patients acutely and 10% long-term. - IH is associated with severe outcomes, including visual loss and increased morbidity and mortality. - The study aimed to evaluate the diagnostic performance, reversibility, and factors influencing normalization of neuroimaging indicators in CVT patients. 👆 Methods: - The study included 26 acute CVT patients and 26 healthy controls. - Patients were classified as having IH based on CSF pressure > 25 cmH₂O, papilledema, or optic disc protrusion on ocular MRI. - Various neuroimaging signs such as optic nerve sheath diameter (ONSD), optic nerve tortuosity, bulbar flattening, and ventricle size were assessed at baseline and follow-up. 👆Results: - Prevalence: 46% of CVT patients had IH. - Diagnostic Performance**: ONSD enlargement > 5.8 mm, optic nerve tortuosity, and pituitary grade ≥ III were the most sensitive indicators, while ocular bulb flattening and pituitary grade ≥ III had the highest specificity. - Reversibility: ONSD and pituitary grade were significantly associated with recanalization. However, some indicators like ONSD enlargement and partially empty sella persisted even after treatment. - Follow-up: ONSD size and pituitary grade significantly improved but did not reach normal control levels. Other neuroimaging signs showed variable degrees of reversibility. ✅Clinical Implications✅ 🎈Diagnosis and Monitoring: - Neuroimaging indicators like ONSD and pituitary grade are valuable for diagnosing and monitoring IH in CVT patients. - High specificity indicators (ocular bulb flattening, pituitary grade) can indicate a low probability of IH when absent. 🎈Management of IH: - Persistent neuroimaging signs post-treatment necessitate continuous monitoring and potentially extended anticoagulation therapy. - The results support the use of non-invasive methods such as ONSD measurement for ongoing assessment of CVT-related IH. 🎈Clinical Decision-Making: - Neuroimaging indicators should be considered alongside clinical presentation and other IH risk factors to guide diagnostic and therapeutic decisions. - In cases of persistent IH indicators without symptoms, non-invasive follow-up methods should precede invasive procedures like lumbar puncture. 🎈Treatment Duration: - Individualized counseling on anticoagulation duration is necessary, especially in patients with incomplete recanalization. - The findings suggest that the optimal duration of anticoagulation remains uncertain and should be tailored based on recanalization status and IH indicators. https://lnkd.in/dHCCTvBH

Meanwhile, tirofiban is frequently applied in emergency situations in interventional neuroradiology (INR). The objective of this study was to analyze the risk profile for the off-label use of tirofiban in INR patients. Methods Data of 86 patients, who underwent neurointerventional therapy and were treated with tirofiban at 2 neuroendovascular centers between January 2016 and July 2017 were retrospectively analyzed. Despite off-label use, recent stroke (< 30 days), recent hemorrhage, thrombocytopenia (< 150,000/µl), activated partial thromboplastin time (aPTT) > 1.3-fold, internation normalised ratio (INR) < 1.5, severe liver insufficiency (Child-Pugh C), and preceding intravenous thrombolysis were considered as contraindications. Results Median patient age was 62 years (range 26–88 years). Patients received tirofiban for extracranial ( n = 35) or intracranial stenting ( n = 35), coiling of ruptured cerebral aneurysms ( n = 6), continuous intra-arterial nimodipine infusion via microcatheters for subarachnoid hemorrhage (SAH)-related vasospasm ( n = 5), or thrombotic complications during neuroendovascular procedures ( n = 5).
