The surveillance and management of intraductal papillary mucinous neoplasms (IPMNs) are guided by several international recommendations, with the most widely recognized being those from the American Gastroenterological Association (AGA), the European evidence-based guidelines on pancreatic cystic neoplasms, and the International Association of Pancreatology (IAP). 💉Prophylactic (perioperative) #antibiotics are rarely needed, however, when we decide to use them we should use them right. Historically beta-lactams were administered 30–60 minutes before the first incision in dogs, then every 90 minutes until the end of surgery. Dr. Ludovic Pelligand from RVC wanted more evidence and conducted a study on the pharmacokinetics/pharmacodynamics of each most commonly used beta-lactams for surgical prophylaxis which helps us choose a more accurate time of dosing for each antibiotic based on expected contamination. The Journal of Thrombosis and Haemostasis (JTH) has published results from the PROXY study, which suggests a fixed dose of four-factor prothrombin complex concentrate (4F-PCC) can safely and effectively be used prophylactically to manage clinically relevant levels of Factor Xa inhibitors (XaIs) in patients requiring urgent surgery. Dr. Dan Witmer, one of our leading hip replacement specialists, recently presented impactful research (alongside Rob Magaldi and Sara Strecker) at the Northeast Arthroplasty Conference, winning 1st place for their poster presentation! Their study focused on the effects of GLP-1 agonists medications on patients undergoing total hip replacement. The key takeaway? These medications can increase the risk of PONV after surgery. Based on this research, our new protocols at BJI and our surgery center recommend stopping these medications one week before surgery to minimize PONV in patients. This research is ongoing and the groups will be re-studied later this year to learn the outcome. #arthroplasty #research #patientcare #OAHCT #MakeYourMove

Very interesting approach to a very difficult subject. Most of the knne pain secondary to a patellar condropathy is a constant challenge for other therapeutic approaches, such as physiotherapy, Orthobiologics and even surgery. In my opinion AMIC might be the way forward in very symptomatic patellar cartilage defects wich are well beyond conservative management. Cartilage repair of the patellofemoral joint is challenging, but AMIC is a robust and effective treatment for retro-patellar chondral lesions, providing significant and sustained improvements in function and pain. This retrospective patient registry analysis by Gille et al. (2023) highlights the positive outcomes of AMIC in the treatment of retro-patellar chondral lesions in 64 patients, with a mean follow-up period of 5.8 years. ⭐ Significantly improved Lysholm, VAS pain and all KOOS domains in the year following surgery, which were maintained throughout the follow-up period.
