Pelvic pain is an all-too-common experience for countless women, disrupting their lives not only during menstruation but persisting during their menstrual cycle. Frequently dismissed as mere "menstrual cramps," this discomfort may signify a deeper issue: endometriosis. This condition extends far beyond routine menstrual pain, often manifesting in additional symptoms like abdominal discomfort, irregular bleeding, pain during sex, back pain, and bladder issues. Despite affecting approximately one in ten women aged 15 to 49 during their reproductive years, endometriosis often evades swift diagnosis. Alarmingly, 68% of women with endometriosis initially receive a misdiagnosis, highlighting the pressing need for heightened awareness and improved diagnostic strategies for this condition. When I see an adolescent or young adult female in my pediatric practice, I always ask: -“Are you menstruating? Let’s talk about the numbers: How often, how long, how heavy?” But the real surprise seems to come when I start asking about the impact on their lives: Does any aspect of your cycle contribute to your missing school, missing sports/clubs/social activities, feeling less capable or less able to enjoy daily life and interactions? Moms often are surprised when I ask their history. I’m curious about the family medical history AND how monthly pain/discomfort/mood is treated in the family. Many families don’t see the discomfort as “fixable” so resilience is the ONLY intervention they see. Let’s look at the lives of teens and #young women with a hope for FLOURISHING. If there is a physical barrier to a high quality of life, including #painfulmenstruation, please tell your physician. Don’t feel you have to wait. Let’s have these conversations. #pubertypain

Teen girls need support for impacts on performance from period pain. #Menstrualhealth modules are woefully missing in #pediatric education. Likewise, on the other end of #reproductivehealth, #menopause, only 1 in 5 gynecologists receive any education. While there is a push to replace doctors with less trained clinician types, less training or education does not serve patients. When hospitals and clinics hire non-physicians who skip years of school and training to learn on the job, they are often being taught by physicians. That takes away from patient care. Patients deserve clinicians with knowledge and training to address these common #womenshealth symptoms. We first need to increase #medicaleducation and #training for women’s health. As long as these health areas are missing then doctors and nurses will not be able to recognize or respond, adding to “medical gaslighting.” Then we need to get women’s health to be valued in clinical settings. Because administrators respond to payment - and pediatrics and women’s health are reimbursed lower - these parts of #healthcare are not valued, literally. It is why pediatric and women’s health service lines are being shut down around the country to shift more operations to what has higher margins for better balance sheets and income statements. Patient care and population health is often secondary to the fiscal health of the institution. We also neee to make healthcare about health and delivering health outcomes.
