HEALTH. Through my own experience of living in a rural community in Northern Ghana, I learned to understand the relevance of SG's mission: “...we take healthcare to the underserved and provide for the needs of the underprivileged” – SG Mobile Health Care, Ghana. Through my own experience I discovered what it means if your health, and even possibly your life, is at stake. Just because you live in a rural community in Northern Ghana. It was a close call for me when I had a severe malaria outbreak (October 2022). Fortunately with a positive outcome. But many stories with a negative outcome are untold. How a story unfolds when it comes to healthcare in rural Northern Ghana depends on several factors: * the availability of transport to the nearest health facility * the quality of the road, influencing how fast you can drive to the nearest facility and if you get stuck halfway or not (e.g. due to flat tyre, water) * the presence and capacity of staff at a facility once you arrive * the availability of diagnostic equipment * the availability of (right) medication * basic, hygienic conditions to support a bit of comfort (e.g. a toilet or running water). STATISTICS: District Wa-West – the home base of SG Mobile Health Care in Ghana - has a population of +/- 97,000 people. 39% of this population has no access to primary healthcare facilities within recommended standards. Republic of the Congo Health System: an endless feast for crooked businessmen and the presidential family. Infant and maternal mortality rates are significant indicators of a population’s overall health and health systems’ effectiveness. In the Republic of Congo, these statistics, with approximately 56 deaths per 1000 live births, mark the State of health disaster in which the country is plunged. A plethora of supervision calls for tenders in direct agreement awarded to the husband of the Minister of Health, a pharmaceutical distributor under the boot of a son of the President, a decree that institutionalizes health apartheid, etc. The entire health system of the Republic of Congo has come under the microscope. It appears to be a portrait of health segregation, of a country with two health systems: one for the enrichment of the President and his followers and another entirely in ruins for the common Congolese.

#CHWSpotlight NASSA Moussa is a community health worker (#CHW) in the village of Tanbogo, in the Ziniaré health district, Burkina Faso. Trained in integrated management of childhood illness (IMCI), Moussa provides essential care to children and pregnant women while educating the community on diseases like malaria, diarrhoea, and pneumonia. "The population is truly happy because they live far from the hospital, and thanks to us, they receive care at any time, even at night." This statement underlines the crucial role CHWs play in communities where access to health services is often limited. In remote areas, CHWs become the first responders, delivering vital care to community members. "With the phone, it’s even better…" Moussa also praises the transformative role of the phone provided by Living Goods, equipped with the Ministry of Health’s mHealth app. "The phone is very useful to us because it makes our work easier," he explains. He adds, "Before, we worked with paper, which was fine, but with the phone, it's even better. With paper, we sometimes had a lot of questions to ask and could get confused, but with the phone, everything is automatic; it’s our guide and reminds us of what we might forget." Nassa also highlights the importance of supervision in his daily work. "Supervision helps us a lot; without it, we wouldn’t manage. In April 2020 there was one death a day from Covid in South Africa, yet we had a massive lock down. What I don't understand is why we as a society are happy with 36 vehicle deaths per day in South Africa. Or even why we are happy with 75 murders a day in South Africa? And there are usually 2,000 total deaths a day in South Africa. Why don't we lock down for all those things? Something went wrong with our ability to think? Was it that the WHO or that the American Health Service or the Chinese Health Services were saying things and the South African Health Service just said, they must know what they are doing? Never mind that health in South Africa is different to in those countries. During the 3 years of lockdown we had hourly stats on Covid sickness, hospitalisations, recoveries, deaths. How come we don't have hourly figures on all cause mortality now? If governments and health departments could provide this during lockdown, they can easily provide it for everything during unlock?

I came across this article on BusinessDay that underscores the harsh realities of Nigeria's despicable state of healthcare. In my last post, I cited Rwanda as an African country with a grass-to-grace story in the healthcare sector. I'd like to highlight a few steps that have helped the Rwandan government revive its healthcare system. I believe some of these steps could serve as action points for us in Nigeria. Envisioning In 1998, four years after the genocide, a new government emerged in Rwanda and launched a consultative process to create a national development plan that focused on the principles of inclusive, people-centred development and social cohesion, codified as 'Vision 2020'. The aim was to move from the mid-1990s disaster towards becoming a middle-income country by 2020. The government also formalized the inalienable right to health in the Rwandan Constitution of 2003. Community-Based Health Systems The Rwandan government's early approaches focused on health equity, access, and accountability. In each village, health workers were elected by village members, and then trained and equipped by the Ministry of Health. Leveraging Technology The Rwandan government leveraged cell phones and new information technology platforms to respond to HIV and challenges in maternal and child health. This led to a 27% increase in facility-based deliveries at the end of the twelve-month pilot phase. Partnerships The Ministry of Health prioritized strengthening the specialized health workforce and partnered with the Clinton Health Access Initiative. This led to the establishment of an academic consortium, a seven-year partnership involving sixteen academic medical centres, six schools of nursing, one school of public health, and two dental schools. The program focused on knowledge transfer, sustained collaboration, and the establishment of other health programs within the Rwandan education system. Participatory Policy-Making Approach Another laudable step is the commitment of policymakers to evaluate novel strategies to improve service delivery. An example is the 'mutuelles de santé' policy, which created a flat premium for all but the wealthiest Rwandans. Through innovative monitoring and evaluation, policymakers have implemented new programs and evidence-based changes to existing ones. Civil society representatives now join ministers in parliamentary meetings to discuss proposals. The Ministry of Health also meets regularly with other ministries to review policies with overlapping interests and harness cross-sectoral synergies. Funding Rwanda dedicated 22.1% of general government expenditure to the public health sector in 2011. This investment has stimulated shared economic growth as citizens live longer and with greater capacity to pursue lives they value.
