Let's talk Non-Responders or Slow Responders This will be part 1 - some of the things I go over first are the actual home program/Recovery Plan and the patient's understanding of it. Are they actually sticking to it? A Big part of my Recovery Plans is temporary avoidance of triggering activities/positions. I think this is often missed or at least not stressed. You can have the best self treatment options available, and the patient is ultra compliant, but they are negating any self treatment effects by one or more activities they do throughout the day. Example of this would be a patient that had a ACL repair, but also had radiating LE pain below the knee that responded to lumbar end range loading. She left 100% better for 2-3 visits, and was able to STAY better for several hours. This is my hallmark of a Rapid Responder. However, she kept coming back saying she was "no better." Eventually her husband told me she sits in long sit for several hours a day crocheting. A simple change positions every 20 min, and a standing table her husband built for her and she was able to stay 100% better. Lastly, if you use repeated end range loading strategies (directional preference), is the patient doing the right direction? Are they hitting end range? Are they dosing frequently enough? I think these two things balance each other out and you don't always have to go into the other more difficult educational/behavioral pieces that I'll get into next post. My first professional job out of uni as I finished my supervision requirements to become a registered psychologist was in child protection. It was 1991, the recession we had to have, and it was a baptism of fire! My whole career I have been grateful for what I learnt in those four years. We knew then that prevention was better than a cure. Benjamin Franklin knew it in 1735 for goodness sake! When he said “an ounce of prevention is worth a pound of cure” Primary Care is THE best bang for our buck. General Practice has been embedded in our communities for good reason, to make it accessible to all no matter where you live. GPs are experts, with at least a decade of medical training. Our modern systems are built on political distraction, and they are resulting in resources spread too thin, undermining primary care. Recently GPs have not been respected or supported as they need to be, to be able to deliver the high levels of care they are capable of to us. Medicare and other systemic issues are undermining their efforts. We as community members can do something about it. Make your frustrations about getting a timely doctor’s appointment really count. As well as venting to your friends and family, tell your local politician about your frustrations. Ask them what they are doing about making sure all Australians have access to a GP Explain to them how you value your GP and what a difference it makes to the quality of your life when you can have an ongoing relationship with your GP and you can get an appointment with them. Tell them about the times when your GP helped facilitate your learning and access to the medical responses you needed. Tell them about what happened when you arrived at the ED because you couldn’t access a GP. Make sure your student can receive emergency health care when they are away for school! Enroll in the ANTHC Purchased/Referred Care (PRC) Student Medical Program before traveling out of state for school. PRC enrollment needs to be done each semester; students need to provide ANTHC PRC the following: - A notarized official letter from the school verifying the student’s full-time attendance. - Patient registration worksheet with current mailing address and telephone number while going to school outside Alaska. - Beneficiaries are required to be screened for alternate health resources. Please call 907-729-5696 to be screened by Outreach and Enrollment. - Once the student has graduated or discontinues full-time status, medical coverage is limited to emergency care for a period up to 180 days. For first-generation/low-income medical students, financial strain continues to take its toll as we approach the holiday season. The graph below (from Early Alert's Population Wellness Dashboard) shows FGLI students consistently reporting lower financial wellness than non-FGLI students across dozens of US medical schools. Here are some comments we received from them: • "I don't make enough money to eat food on a regular basis after having to leave my job in the dorms" • "Wasn't able to get financial aid to support some of my basic needs" • "I don't have a job, and I will eventually run out of my savings" How are your schools supporting FGLI students? Would you invest a decade in education just to scrape by? 😶 I'm sure your immediate answer is no but, that's the reality for many clinic owners. They're taught to rely on one income source that often backfires: insurance🤒 Why it backfires: 1. NEET Leak 2024: A Pyrrhic Victory for Cheaters, a Looming Threat to Our Health The news of the alleged NEET UG 2024 paper leak leaves a bitter taste. While some may see it as a shortcut to success, the true cost falls far heavier on the future of our healthcare system and, ultimately, our lives. Cheaters Win (This Time)- Those who gained access to the leaked paper undoubtedly have an advantage. They may get a higher score and secure a coveted medical seat. But this "victory" is hollow. Their foundation will be built on shaky ground. They'll lack the deep understanding and critical thinking skills honed through genuine study. The Real Losers: Future Patients. Imagine a doctor who breezed through medical school thanks to cheating. Will they be able to make critical diagnoses under pressure? Will they possess the knowledge and confidence to handle complex medical situations? The answer is chilling. Our health, in such scenarios, becomes a gamble. A Threat to Us All- A single doctor with a compromised foundation can impact countless lives. Multiply that by the number of potential cheaters who benefit from the leak, and the threat becomes alarming. The integrity of the entire medical profession is at stake. What Can We Do? We need a strong response. A thorough investigation must weed out those involved in the leak. Stronger security measures are crucial for future exams.
