Transitional Care offers the structure and stability these young adults need to begin building a life they can be proud of. 💙 The transition from adolescence to adulthood can be daunting for anyone, but for young adults aging out of foster care, it presents unique challenges. Programs like Palmer Home’s Transitional Care are crucial because it fills a gap that often exists for young adults who are no longer eligible for foster care but are not yet ready to be completely independent. The goal is to improve outcomes for children in foster care, including those in Community-Based Care areas, while recognizing + compensating caregivers for delivering high-quality services.
