The reality is our health care system is in crisis. Millions of people without care, long waits, preventable diseases becoming fatal all while we aren't tapping into resources that are right in front of us. There's enough work to go around. We don't need to compete with other professions on who has more value. We all have the skill and ability to make care better for Ontarians. We all deserve that. This article isn't just an opinion. It's a fact, there's TONS of research that shows NPs offer safe, effective, high quality care. How many of the 2.5 million unattached patients currently are or could have access to care through an NP? This is such an unfortunate situation in Tillsonburg and one that highlights the differences in how family doctors are recruited, supported, and onboarded into practices in Ontario. Tillsonburg is just outside my recruitment area so I felt helpless when the doctors planning to leave contacted me to ask for help finding replacements. They have also reached out to the neighbouring recruitment team in Woodstock. Family doctor recruitment is not managed provincially in Ontario. Each county/ town/ city has to create and fund this position themselves. In many cases the path to having a family doctor recruiter starts by a team of healthcare leaders advocating and pitching the municipality or health region for support. Tillsonburg is experiencing this drastic family doctor shortage without having a recruitment person in place to assist with finding and onboarding new doctors. When this is the case, it falls on volunteers within the medical community to manage the recruitment tasks. Having discussed this with the people in that volunteer role in London before our recruitment program started, they stated not having a recruitment lead here was making it hard for doctors to practice in this area. Recruitment leads have to continually fundraise from their OHT, hospitals, business community/ Chamber of Commerce, municipality, community foundations, developers, business leaders, etc to keep their programs going. They raise funds for their programs which includes their pay, travel, and materials and most also need funds for incentive money for newly-signed family doctors who are choosing to start practices where they can get financial relief from their student debt and moving costs. As different communities have different abilities to provide these funds, there is a disparity between areas with a recruiter and incentive funds and without. Tillsonburg has neither. *The physician recruiters of Ontario have self-organized into a group called Ontario Physician Recruitment Alliance and under the leadership of Jill Croteau, have created a job board with family doctor postings from across the province. (This is new so not all the areas have their postings up yet). www.opra.ca -The GTA does not have family doctor recruiters and this task is typically handled by clinic owners.

Due to an aging population [1], many patients in their last months of life are presenting to #emergencyDepartments (ED), often in the final stages of serious illness. A #QualityImprovement (QI) project led by Julie Lockington and team @ Vancouver Coastal Health embedded a #Palliative Clinical Nurse Specialist (CNS) in the ED to proactively identify patients with #palliativeCare needs, facilitating #goalsOfCare conversations and connecting #patients to appropriate supports. In the trial, the CNS has helped prevent 22 acute admissions, saving an estimated $680,000, while improving #patientCenteredCare and reducing ED stays. The project is runner-up in the recent BC Quality Awards' "Coping with Transition from Life" category [2]. The BC Quality Awards (established in 2009 by Health Quality BC) are an annual celebration of people and projects that improved the #qualityOfCare in British Columbia. As you may or may not be aware, Julie is one of the masterminds behind many QI initiatives such as the Wobble Room [3] and the ICU Wishing Well [4]. Julie is also a co-investigator in the collaborative #research program between The University of British Columbia and Rogers Communications, playing a key role in a #healthcare research on Timely and Extended Monitoring in Emergencies and Disasters [5].
