The use of an ambulatory surgery center (ASC) has allowed some surgeries to become fully outpatient, with patients going home same day. This is a significant cost savings to the patient and reduces the anxiety that can surround surgery and a hospital stay. Efficient - more streamlined processes 3. Convenient 4. Reduced infection risk - surrounded by less sick patients and shorter stays 5. Specialization - ASCs can often have a focus on specific procedures/specialties 6. Patient satisfaction - offering a more comfortable and personalized environment often leads to higher patient satisfaction levels Personally, I think this is a universal healthcare shift that I agree is having an "uptick". The Importance of Patient Preparation Before Orthopedic Surgery? Today, we'll discuss a crucial concept in orthopedic surgery: Preoperative Optimization. What is Preoperative Optimization? Ensuring Overall Patient Health: When a patient comes to you with a condition that doesn’t require immediate surgery—whether it can wait a month, six months, or even a year—it's essential to conduct thorough preoperative optimization. This means preparing the patient comprehensively to ensure the surgery goes as smoothly as possible. Necessary Tests and Examinations: The first step involves conducting essential tests such as HBA1C, kidney function tests, liver function tests, and complete blood counts. It's vital to confirm that everything in the body is functioning normally before the patient undergoes surgery. Why is Optimization Important? Improving Surgical Outcomes: For instance, if you find that the HBA1C level is 11, don’t proceed with a Total Knee Replacement (TKR) and plan to manage the sugar levels before and after the surgery. Such a patient will likely have worse outcomes; therefore, it is crucial to stabilize conditions before the surgery. Reducing Complications: If the patient is a smoker, they need to quit smoking for at least a month before the surgery. If they consume alcohol, they should stop for six months before the operation. These factors significantly affect surgical outcomes and recovery speed. Steps for Optimization: 1- Conduct Necessary Tests and Examinations: Ensure all tests and examinations are normal. 2 - Adjust Medications: Ensure the patient is not taking any medications that could impact the surgery. 3 - Advice on Smoking and Alcohol Cessation: Instruct the patient to quit smoking and alcohol consumption well before the surgery. Proper patient preparation before surgery is vital for improving surgical outcomes and reducing complications.
