Deep Dive 🌊 – Skilled Nursing Facility (SNF) Medicare Improper Payments & the risk to #medsupp Skilled Nursing Facility (SNF) claims under Medicare Part A continue to drive significant improper payment losses. The 2024 CERT report revealed $5.9 billion in improper payments (17.2% improper rate), a sharp increase from $4.8 billion (13.8%) the prior year. Why does this matter for #Medigap? Most Medigap plans provide some coverage for SNF. Medicare covers SNF for the first 20 days. Medigap typically provides some coverage for the next 80 days. After 100 days, individuals are responsible (with caveats, such as restored benefits for recovery). What’s the risk? 😱 The 2024 CERT data revealed these top root causes for SNF improper payments: Case Mix Group (CMG) documentation missing: 388 HIPPS level recoded based on documentation*: 269 Order missing: 176 Physician’s Certification/Recertification inadequate: 80 Documentation to support level of care missing: 71 CMG documentation inadequate: 41 Illegible signature without log: 39 Other: 33 Physician’s Certification missing: 31 Order inadequate: 24 Note: Only one item (*) is frequently associated with partial improper payments. The rest appear to involve fully non-compliant claims. What’s the impact? 🔨 Even though Medigap may cover only a small portion (e.g., ~$200/day), the sheer volume of non-compliant claims poses a substantial financial risk. Based on CERT findings, 764 claims shouldn’t have been payable at all, potentially spanning both Medicare-covered days and Medigap-covered periods. Second Opinion? 🤔 Adding complexity, the MACs’ targeted probe-and-educate audits report a much lower 7.28% error rate—a significant discrepancy with CERT findings. MAC TP&E Results Snapshot Timeframe: July 1, 2024 – September 30, 2024 Claims reviewed: 1,277 Error rate: 7.28% Top errors: Missing or insufficient certification/recertification documentation Lack of medical necessity for SNF services Failure to meet "daily" skilled services requirements HIPPS recoding due to inadequate documentation More details: https://lnkd.in/gsrUB3pX Long Story Short: 📖 While Medigap’s liability for SNF claims may be limited, the prevalence of claims that shouldn’t have been payable at all signals a major financial risk. It’s worth taking a closer look and questioning when 2+2 doesn’t add up to 4.

Now that I’ve spent the last few years building this innovative practice from scratch…I’ll offer some helpful insight. Let’s get into the nitty gritty on billing codes that are utilized in nursing facilities (and skilled nursing facilities). I’ll try to keep this very simple. The specific codes that can be utilized as “incident-to” in this setting (for long term care ONLY) are 99307, 99308, 99309, and 99310. With 99307 representing the lowest complexity and 99310 representing the highest. The higher the complexity, the higher the reimbursement. The people that live in these facilities have several chronic disease states, and as a result, nearly every one of them meets the criteria to justify billing for the two highest billing codes. One of the criteria for billing a higher complexity is that the assessment resulted in an intervention. Why is this relevant? Because we have a health care system that is VERY dependent on medication based interventions. So, prescribers are incentivized to write orders for more meds whenever they have a visit. How can pharmacists make an impact? When we work collaboratively as an employee of the provider, we can identify interventions that result in eliminating unnecessary meds, and educate the residents (and/or family and staff) about how polypharmacy is contributing to many of the complaints or problems they experience. As a valued member of the medical team, we are also able to have open dialogue with the prescribers to identify new, more beneficial interventions that are not adding more medications. There are dozens of examples. But here is a simple, and frequent one. Fenofobrate is quite frequently started, because lab providers are still listing triglyceride levels over 200 as abnormal or elevated. But new evidence shows that Fibrates should only be utilized as an adjunct agent if triglycerides remain over 500 after 8 weeks of therapy on a different cholesterol lowering agent. In the last five years I’ve only seen two cases where I ended up keeping fenofobrate on board long term. Rather than starting a new med, educate the resident that triglycerides are directly related carb intake. If they are consuming several different carbohydrate rich substances try to get them to commit to cutting one of them out. Make that your intervention instead! The ideas and opportunities for pharmacists to shine in these collaborative models are countless and I’m still finding more items like this as we continue to grow, and collaborate with more providers!
