Most of the conversations around GP access have focussed on the supply and speed of access to appointments, but this limited view hides some of the other, important issues that influence people’s experiences of access. Access to GP services is a dynamic process and in this podcast, Nada Khan and Carol Sinnott, GP and THIS Institute Senior Clinical Research Associate, talk about a recent study which takes a deep dive into the literature around access to general practice to see if the candidacy framework would be a helpful tool to help identify and address some of the problems with access to GP services. I hope to listen to this. Productivity is not a term I associate with unpaid family caring, unless you aim to change incontinence pads faster for the same zero money. It doesn't capture the love I had for my late mum in caring for her, nor the affection I had for the paid carers who became my extended family during that period of home care during a pandemic. It's also not a term which sits easy with me with my neurodiversity hat on - it's a difficult term in association with ADHD, where us neuroatypicals don't easily fit into tight structural constraints, and can only flourish when we want to. It is of course an incredibly useful term in neoliberalism, where your value is a function of you appearing on a balance sheet. Payment by results is basically activity based costing. Health in diabetes is not valued as highly as the number of cataract operations, episodes of dialysis, retinal blasts for diabetic retinopathy, amputations done by the vascular surgeons, and so on. So long as people can't see their GP, none of this is addressed anyway. Rant. Over.
