Case History: This is a referred case of retained root stump of 37 post a failed extraction. The patient complained of pain after a failed attempt of extraction of 37 in some other clinic 2 weeks ago. On Clinical Examination it was found that retained root stumps were present w.r.t 37. On radiological examination it was found that both mesial and distal roots are curved with adjoining 38 mesially impacted. IAN proximity was present. Successful extraction was done by trans-alveolar method taking care of the arc of rotation and by not damaging the Inferior alveolar nerve.

This is an image of a Scleral Lens that a patient came in wearing at a subsequent evaluation. In these cases, it is useful to be able to evaluate clearance without fluorescein in the bowl of the lens as you can assess the lens fit after several hours of wear quickly at the slit lamp without the use of OCT. I have found this to be very beneficial for clinical efficiency. Sodium fluorescein was applied on top of the lens to confirm there is no leakage in the periphery to ensure no impactful edge lift. Again, another quick technique utilized at the slit lamp not requiring OCT to help evaluate the Scleral haptics

Dear Colleagues, A recent publication in the American Journal of Case Reports by authors from the University of Texas at Austin, Austin, TX, USA, highlights the successful extraction of an embolized iliac vein stent from the right heart via a surgical approach. This case report emphasizes the importance of considering alternative methods when percutaneous retrieval fails. With an estimated incidence of 1% to 3%, embolization of peripheral venous stents remains a feared complication. The findings suggest that the surgical approach can be feasible without requiring tricuspid valve repair or replacement. Seeking help regarding a challenging case of recurrent in-stent restenosis that has proven resistant to conventional management. The patient has experienced three episodes of in-stent restenosis affecting two different coronary territories, despite sequential trials of multiple P2Y12 inhibitors (Brilinta, Effient, and Plavix) with confirmed good compliance. IVUS evaluation demonstrates optimal stent deployment with good expansion, proper wall apposition, and no identifiable mechanical issues. Despite optimized medical therapy and proper stent deployment, we continue to see recurrent ISR. What is your suggestion ?
