We developed a novel technique called MCP (Meticulous Cauterization Protocol with Apical Preservation) for Robotic Prostatic Aquablation that allows: 1) this procedure to be done as an outpatient procedure (no hospital stay required), 2) crystal clear post-op urine (no need for traction or continuous bladder irrigation), 3) great flow with a huge prostatic opening, and 4) apical preservation to try to preserve ejaculatory volume. Cornea Update: Endothelialectomy With Trypan Blue Staining to Enhance Descemetorhexis Visualization: Purpose: To describe a technique involving combined endothelialectomy and trypan blue staining to allow for improved visualization and Descemet membrane (DM) removal during endothelial keratoplasty. Methods: Endothelialectomy with 2 disposable endothelial irrigating cannulas (Vortex and Sterimedix) and an irrigation-aspiration handpiece are described. Several passes over the desired area are made to ensure adequate endothelialectomy treatment. Trypan blue is instilled into the anterior chamber, and after irrigation, the trypan stains the exposed DM, which enhances visualization. Subsequently, one's standard technique for descemetorhexis can then be performed. Results: Endothelialectomy with trypan blue staining was performed in 9 eyes and was able to successfully enhance visualization of DM in all cases. The technique easily allowed for visualization of the descemetorhexis edge despite the poor view. Although all 3 instruments for endothelialectomy allowed for successful descemetorhexis, the Vortex endothelial cannula provided more complete and consistent endothelialectomy with fewer passes. The descemetorhexis tissue from 1 case using the Sterimedix cannula was sent for histological review and demonstrated a paucity of endothelial cells, intact DM, and blunted guttae. Conclusions: Targeted endothelialectomy and trypan blue staining can be a useful technique to enhance visualization during endothelial keratoplasty. Routine Endodontic case done in FUE microscopic center. ♦️upper left second premolar♦️ 🟥A 40 years old patient came to our clinic complaining from severe intense spontaneous pain related to his upper left posterior area with inability to eat at the associated site. After clinical and radiographic examination: widening of periodontal ligament space related to the involved tooth Clinical Steps : 🟥access cavity preparation. 🟥Rubber dam isolation and field disinfection. 🟥 Build up of the mesial missing wall 🟥#10K files were used to check patency and working length determination. 🟥Working length determination using Apex locator and radiogaphic confirmation 🟥canal is cleaned and shaped using rotary files 🟥Continous irrigation with sodium hypochlorite 5.25% during the procedure to disinfect the organic part 🟥Then irrigation with EDTA to disinfect the inorganic part 🟥Followed by irrigation and activation by sodium hypochlorite again as a final flush 🟥Using sterile paper points to dry the canals. 🟥Obturation was done by vertical compaction technique and resin based sealer. #endodontics

✨ Successful Root Canal Treatment on an Upper first Molar 🦷 Today, I had the rewarding experience of relieving a patient’s pain and restoring their tooth with a complex root canal treatment. 🔍 Case Details: A 23-year-old patient presented with severe spontaneous pain in the upper right posterior region, persisting even after root canal treatment done three months ago. After clinical and radiographic examinations, we identified a missed MB2 canal as the source of the problem. 💡 Clinical Steps: 🟥Access cavity preparation. 🟥Rubber dam isolation and field disinfection. 🟥Rebuilding the missing wall to restore the tooth’s integrity. 🟥 #10 K files used for canal patency and working length determination. 🟥 Working length confirmed with an apex locator and radiographs. 🟥 Canal cleaning and shaping with rotary files. 🟥Continuous irrigation with 5.25% sodium hypochlorite, EDTA, and a final sodium hypochlorite flush for disinfection. 🟥 Drying the canals with sterile paper points. 🟥Obturation performed with the vertical compaction technique and resin-based sealer. The use of advanced techniques, including thorough irrigation and microscopic precision, ensured the missed canal was located and treated effectively. 📚 Takeaway: Every challenging case is a learning opportunity.
