As stroke is the no. 1 leading cause of permanent long-term disabilities and counts for approximately 46$ billion in the United States, research is important to improve clinical outcome for patients. Yet a number of studies have shown that using a single system to diagnose and treat patients in the angio suite can greatly accelerate treatment and improve outcomes. To evaluate the diagnostic accuracy of non-contrast flatdetector CT compared to multi slice CT an investigator-initiated multi-center clinical trial called SPINNERS was initiated by Marios Psychogios, Prof. Dr. from University Hospital Basel in Switzerland and Adam Arthur from Semmes Murphey Clinic in Memphis, USA. If the outcome of the study shows non-inferiority of flatdetector CT, this will support the one-stop-management (or angio only) approach which may improve diagnosis, door to needle time and, subsequently, the outcome for acute ischemic stroke patients. This step in research is an important milestone to lift stroke treatment to the next level. Kudos to the two investigators for focusing on this meaningful topic.

The #OneStop management approach for acute ischemic #stroke patients leads to shorter times in the hospital and better functional outcomes. The latest study from Manuel Requena et al. has proven that a one-stop approach is additionally cost-effective compared to the standard workflow. What is missing at the moment is a study proving the high #sensitivity and #specificity of FDCT in the detection of cerebral hemorrhage, compared to the gold standard MDCT. Differentiating ischemic from hemorrhagic stroke allows us to initiate stroke treatment with thrombolysis (IVT). We at the University Hospital Basel have started, in cooperation with Semmes Murphey Clinic in Memphis, USA, and Dr. Adam Arthur, the #SPINNERS trial. With this study, we want to prove that an angiography system with Sine Spin technology is the right tool to diagnose and treat acute stroke patients.
