More isn't always better. Or is it? That's the problem - sometimes it is and sometimes it isn't, and generally we can never know unless we are truly "inside the deal". Scaling isn't simple - important nuances of the winning model upon which the future is built are lost and headwinds generally grow during the scaling timeframe. What erks me is that in the end, you don't usually have alot of the secret sauce you started with...the secret sauce needed to change from sickcare to healthcare perhaps? Remember some of the legacy models CareMore, XLHealth that for the most part disappeared after acquisition. I would argue ChenMed is one of the few (if not only) Primary Care models that scaled successfully. Until of course HCC v28 took effect this January and once Walmart Health Virtual Care takes hold, assuming they do. We at BP2 Health have been on a 30 day sprint for a client who has an unadulterated vision for a spin on Oak Street Health and ChenMed. I'm a polyanna so I'm always game for something new. However, it's been interesting to balance optimism with my battle scars from years of helping clients deal with #medicareadvantage to build the value prop and financial model. And as I build out the cap tables I relive the carnage I've witnessed with each new round of equity in bygones. Don't worry, I'm still hopeful - if not, that means I've given up on our industry and that's just not an option. What I've learned of late is that we MUST bridge the gap between all the shiny objects on the #vive2024 and #himss24 exhibit floor with the bricks and mortar to move our system from #sickcare to #healthcare. I think we need both. We need REAL change. Our tagline from day 1 almost 8 years ago at BP2 Health is "your best chance for real change" - there is nothing I want more than to work with those who want to make real change happen not just chase the $$. I'm calling all REAL change unicorns (I'm even tagging a few so they know their unicorn status is duly noted)! Oh and did I mention I'm on a mission with our podcast (The Reverse Mullet Healthcare Podcast) to find as many of the unicorns out there as possible. I recently wrote a piece analyzing Oak Street Health's growth from company inception to today. Long story short: CVS wants to juice OSH clinics with Aetna members and use pharmacists & pharmacy techs as extenders for OSH members (side note: if they can retain the pharmacists?) The question is whether CVS can effectively execute on the OSH clinic strategy and shorten the time it currently takes to make senior advanced primary care clinics profitable. CVS wants to open 50-60 new clinics a year through at least 2026. In 2024, CVS priced its MA plans purposefully low to maximize member growth, adding 800,000 lives. This strategy means they believe in the OSH-Signify-Pharmacy flywheel long-term. To me, Oak Street Health’s high-touch clinic model with high-needs, chronic patients is the linchpin in the flywheel to drive overall organizational success by keeping patients within the CVS model across health plan, PBM, pharmacy, and care delivery. Today, pharmacies nationwide dimmed their lights to protest the significant challenges they are currently facing. This impactful gesture, championed by the National Pharmacy Association (NPA), highlights the urgent need for greater recognition, understanding, and action. Pharmacies are essential to our healthcare system. Offering critical services and support to communities everywhere.
