Tony S. Shen, MD, hip and knee surgeon at HSS, presented “Using Machine Learning to Predict Need for Hospital Admission after Total Hip or Total Knee Arthoplasty” at a session today at the American Academy of Orthopaedic Surgeons (AAOS) Annual Meeting. According to Dr. Shen, as more joint replacements are taking place in ambulatory settings, safety must be the priority. Today, institutions have specific criteria that work for them. What we are trying to do is organize and analyze data and find patterns that will help us predict who would benefit the most from a hospital stay and who be truly safe to go home. This study is a starting point, but in the future, we will have better tools, better data, and better ways of knowing what the best practice should be. #AAOS2024

Chronic/Neglected Achilles tendon rupture is a limiting condition we have to deal with in our clinical practice. Currently, there is no consensus on the approach and techniques to treat this pathology. ESSKA - AFAS conducted a survey that aims to identify the current practice of specialized foot & ankle and sports traumatology surgeons. Referral-to-treatment (RTT) #data has been collected for well over a decade and, in the early days, the Other category was a catch-all closely followed by trauma and orthopaedic (T&O). The top 5 #specialties didn't change until as recently as Q1 2021/22, when the Other category was split into four separate categories of their own. Surgical specialties tend to get most of the press and national focus - T&O, general surgery, ENT, etc. However, in the top 15 specialties, it is #medical specialties that dominate. It cannot be forgotten that these need as much focus as surgical specialities to reduce waiting lists to ensure patients in these specialities aren't left behind. Let me present a piece of supporting evidence for Benjamin Schwartz, MD, MBA’s observations. At the ISTA 34th Congress last week I was sitting 1 row behind the Editor-in-Chief of the British Joint Journal, Dr Fares Haddad. The exact context of the discussion I can’t relate perfectly but the comment he uttered regarding papers/data was “90% is garbage”. I share a cynicism around the mighty p value and Big Data drivel but this frank admission shocked me. WOW. I have experienced being on both sides of the peer review process and reviewers are honest but often not experts or experts with blinders, limited to the status quo or statistical machinations. This may work for a Mayo type institutional database but people learn by sharing ideas and ideas are what changes the course of incremental progress. This exchange occurs in hallways, tables, and pubs at meetings. I often wonder about this term, “Value based Care”. From the surgeon perspective, it has undeniably translated to “Do more for less”, which is a bad strategy unless you’re a Pharoah too. From a facilities perspective it’s good as with fixed payments, lowering costs and increased volumes produces revenues. Thus, the surgeon ASC owner incentivized to churn and burn but hopefully not burnout. Finally, the insurers, who it really doesn’t matter, as they own the playing field and governmental agencies setting rates and policies entirely. They just move the pieces around, guaranteeing heinous profits at the expense of a citizenry that, to them, pays more to be ill than healthy. Value here would be to invest in wellness and have capped profits as a healthy, financially sound country, unhampered by bloated and denying for profit insurers, is a more productive country. Lots to consider But let’s thank the Journal of Orthopaedic Experience & Innovation for providing a forum for open discussion of topics not typically found in traditional publications. Hey, don’t these “traditional” publishers make outlandish profits on the backs of researchers too? It’s a Mad Mad World, isn’t it? Guess it’s always been somewhat unfair. #followthebod #Bodsquad
