In the ever-evolving landscape of healthcare, one crucial aspect often remains underemphasized: advanced care planning (ACP). Integrating ACP into primary care is essential for fostering patient-centered care and enhancing the overall quality of health services. While these conversations can be uncomfortable, they are a crucial part of a holistic plan of care for our patients. As I think back over the conversations and cases Christina Rama, MD, MMM led around this topic, I am so grateful for her providing me and my teams the tools to implement ACP into our conversations with our patients. Here’s why this integration matters: Holistic Patient Care: By incorporating ACP into routine check-ups, we ensure that discussions about future healthcare preferences are part of the ongoing dialogue about health and well-being. In the NHS and in many health systems around the world, demand for health services has begun to outstrip health systems’ capacity to provide care. And yet we still see large numbers of acute beds being occupied by patients who are medically fit and could be discharged to their homes or residential and nursing care homes. So what’s stopping this happening? This is the question that myself and my global KPMG colleagues looked at in some detail a few months ago – if it was an easy thing to fix, it would have been fixed already! We set out to think differently about how to help health systems deliver better value for the communities and the patients they serve. We used the KPMG Connected Enterprise for Healthcare approach to redesign care pathways and to leverage digital solutions and new ways of working, such as improving efficiency of critical transitions in care pathways. For far too long, the healthcare system has been guided by corporate priorities, what we call ‘institutional first’ healthcare, while patients have faced limited access to timely, affordable care. The Patients First Foundation is being launched because it is time to shift control back to patients, putting ‘patients first’ and their well-being at the center of every decision, and ending corporate ownership of our health. I’m Steve Carbonara and I am going to be sharing my personal health journey over these past 5 decades in order to shed a light on what is wrong with our system, both from a patient and healthcare technology insider point of view. I am looking for the next One Million Patient Stories, from all of you, to prove how common these issues are and learning which are the most costly or deadly problems we need to solve first. Together, we can begin to build a process to hold every stakeholder accountable for their actions moving forward.
