Broach Handle - Stryker Orthopaedics Heavy, overbuilt, all-stainless, hard angled, slippery. Surgeons felt that Stryker’s broach handle was a relic from an era when instruments were (and many still are) exercises in mere functionality. The broach handle is a deceptively tricky instrument since it’s handled in a variety of ways throughout different scenarios. As with any instrument, it should be easy to identify, remove, and pass from its instrument tray between nurse and surgeon; as a reamer, it needs to provide a secure, comfortable grip while wet and under strain; as an inserter, it needs to protect the surgeon during impaction/extraction while providing a generous striking target; it needs to provide an intuitive, easy-to-operate quick-connect for a variety of attachments. A state-of-the-art facility with expertise in orthopedic, spine, dental, surgical robotics, and medical devices needed to upgrade its dust collection system within its production facility. The application was for a sanding and buffing operation. The pedestal sanders and buffers were connected to an undersized and inefficient dust collector which resulted in fugitive dust escaping the system. This dust would settle on finished parts and work surfaces and cause the shop to look dirty. Hastings installed an IVEC cartridge dust collector ducted to five drops on double-wheel pedestal buffers. Modifications were made to each buffer to add better hoods and duct connections to ensure the dust was contained and captured. Each pedestal triggers an IVEC gate to open and start the dust collector. The dust collector operates automatically and modulates based on filter pressure and demand from the buffers. To protect the people and facility, a spark cooler was included in the system design. The result is a much cleaner room and no dust escaping the process. The system has been installed for five years and the customer is pleased with the performance and filter life. Stryker Orthopaedics - Triathlon - Total Knee Replacement We began in the OR, recording multiple surgical procedures and interviewing everyone with a stake in the instrument set: surgeons, scrub techs, nurses, sales staff, etc. We deconstructed the surgical procedure, step by step. enabling us to analyze each instrument and determine its value: is there a need for redesign, alteration, refinement, combination, omission? Weeks of intense sketch ideation followed. Concepts were debated, refined, then developed as semi-functional sketch models. Ergonomics and aesthetics took shape shape for design team and surgeon evaluation. Models were refined, then developed in CAD for semi-functional prototypes, providing a greater degree of feedback while also enabling detailed manufacturing discussions. Sawbone labs followed, and the more complex instruments were developed as cut-steel prototypes for cadaver lab testing. As feedback came in, CAD geometry was refined, FEA on critical components conducted, tolerance studies performed, and manufacturing drawings initiated. What are your thoughts when you see this? It’s only a matter of time until this is implemented everywhere. This extremely thin tissue example in the video below, makes me imagine the obvious benefits of Roller Based Suturing used in the small intestine or even vascular surgery.
I really did not think this was possible considering I am just engineer with no laparoscopic training. The tissue was insanely thin … ten thousands of an inch … and the sample was so small … I had to 3D print a fixture to hold it. I visualized in my head how I wanted things to go but thought it was just a dream; nevertheless, I set up the camera just in case. And then, OMG, OMG, OMG I did it on the first attempt … which was good considering I only had one tissue sample … I figured I was going to have to order more. I am so excited to share this, and I continue to be amazed at how easy roller based suturing is. I think my next step will be to learn how to tie knots. If you know how, it is easy to do with this device and we have had many surgeons do so in labs. Knot tying is basically the same as using a standard needle driver. It is the needle handling, as shown in the video, that is so much easier, safer, and faster.
