The Royal Flying Doctor Service of Australia released its second annual Best for the Bush Rural and Remote Health Baseline 2023 report today, in which paramedics are recognised as 1 of 3 clinical groups that should be supported to work to their full scope of practice. "The RFDS recognises the importance of healthcare providers being able to work to their full scope of practice, particularly in rural and remote areas where there are a wide range of workforce limitations and shortages. We note three clinical groups that at this current time would particularly benefit from further effort to support them working to their full scopes of practice: nurse practitioners, paramedics, and social workers. For paramedics, further work is required to identify relevant roles and opportunities to determine how this skill set can be used beyond ambulance services. This includes in aeromedical retrievals as well as community paramedicine. Additionally, a dual-registered nurse/paramedic role is emerging and appears to be well suited for remote Australia." THEY’RE EATING THE DOGS! Fun interview with Mary Chiarella in australian doctor today. TLDR? Nursing advocate provides heavily biased opinions in favour of further funding and expansion of nursing roles. That’s advocacy-nothing new. The last question however is probably the one that I really wanted an answer to - Mary Chiarella I would love to hear why the government shouldn’t just fund free walk in clinics staffed by doctors? Back in the bad olden days the Medical lobby groups would have a conniption at the thought of being salaried or funded by government. But younger GPs are less and less interested in running their own for profit businesses. Among my peers I can think of a huge number who would be quite happy to have a salaried role and provide care in a quality and meaningful way rather than a fee for service service treadmill. So why are we directing all this funding to nurse lead pretend GP clinics and not having a conversation about using actual general practitioners to provide actual general practice care? If it’s because nurses are cheaper then we should be having that discussion with the public - nurses are cheaper and that’s why you get “free” (completely taxpayer subsided) care from a nurse led clinic. If it’s because of workforce well it’s hardly like we have a plethora of nurses lying around - every workforce prediction report says the same thing about nurses as it does about GPs… So why?
