All hormonal contraceptives are pseudo-hormones that are molecularly different from your natural endogenous hormones. They are technically and officially classified as endocrine disruptors. They literally shut down ovarian hormone production and replace naturally produced estradiol and progesterone with endocrine disrupting chemicals. In contrast, modern hormone replacement therapy delivers molecularly identical estradiol transdermally so that it enters and acts in the body as estradiol. It is identical to the ovarian-produced estradiol and either supplements or replaces ovarian estradiol. The levels may be different, so in that way, it's not a perfect match, but molecularly, it is identical. And it's the same for oral progesterone. You really can't compare hormonal birth control to bioidentical HRT, but if you, remember that it's like comparing fruit-flavored jelly beans to actual fruit. Most people know that smoking and drinking alcohol during pregnancy is not ideal. Yet very few know just how important it is for both parents to maintain healthy behaviours, not only during pregnancy, but in the weeks or months beforehand. A range of modifiable preconception behaviours can improve maternal and #ChildHealth outcomes - such as prospective mums and dads being physically active and maintaining a healthy weight. But a lack of publicly available information, health professional’s knowledge, and time in health consultations means many Australians are in the dark about the importance of #Preconception health. Tonight's journal club will be discussing how to work with surrogate decision makers. Surrogates make approximately 75% of decisions for those hospitalized with life threatening illness. One of the key take homes is the importance of clinician communication and the need for a "point of contact" to coordinate the communication and support surrogates who are at risk for anxiety, depression and PTSD. Medical training focuses on the individual patient with less preparation for working with surrogates. Palliative care emphasizes the family as the unit of care; supporting both patient and care givers.
