A recent incident involving an Agusta A109 helicopter during marine pilot transfer operations sheds light on crucial safety protocols in helicopter landings on vessels. The incident, detailed in an Australian Transport Safety Bureau investigation report, underscores the importance of aligning procedures and ensuring clear visibility of potential obstacles during such operations. The Agusta A109E helicopter encountered a tail rotor strike on the bulk carrier Tai Keystone due to an undetected handrail on the ship's landing site. The investigation found discrepancies in the vessel's helicopter operations checklist, which led to the handrail not being removed as required for safe landings. Stuart Macleod, ATSB Director Transport Safety, emphasizes shared responsibilities, stating, "Vessel operators should ensure their procedures and landing areas on ships are aligned with relevant guidance material." He also highlights the need for visible markings on retractable obstacles to aid pilots in detecting potential hazards. Since the incident, both the ship's operator and the helicopter operator have made significant procedural updates to enhance safety. These include checklist revisions and providing visual representations of landing sites to pilots before departure. I commend the Brazilian authorities for their timely and comprehensive preliminary accident investigation report. Shows the criticality of accident investigations to share, industry wide, the causal factors of accidents to avoid future occurrences. International Air Transport Association (IATA) is focused on better reporting of commercial aircraft accidents from the 49% identified within latest #IATA_Annual_Safety_Report.

🚁 Helicopter Crash at Bonny-Finima: A Call for Accountability and Safety in Offshore Logistics 🚁 Tragedy struck on October 24th, 2024, at 11:22 am as a helicopter en route to FPSO NUIMS Antan OML 123—previously run by Addax and now operated by NNPC—crashed, claiming precious lives. This helicopter, manufactured in 1997, was reportedly scrapped and deregistered in 2018, yet it continued flying until 2024—27 years later. This reflects a devastating lapse in lifecycle assessment and enforcement of aircraft obsolescence standards. It was a tragedy waiting to happen. 💔 As an offshore worker, I’m deeply saddened and outraged by this loss. Offshore workers already face immense risks; logistics should not add life-threatening dangers. Before boarding, workers deserve transparency on the integrity of the helicopters they rely on. It’s critical that we ask questions: Inquire about the helicopter’s maintenance records and last inspection date. Request information on its age and the manufacturer’s recommended operational lifespan. Push for clear regulatory approvals from governing bodies that have recently certified it for safe operation. I’m lending my voice to urge regulatory agencies to investigate why an expired helicopter was still in operation and to prosecute if necessary. Obsolete aircraft must be retired, and standards strictly enforced to prevent tragedies like this in the future. I call on these bodies to push for zero accidents in aviation safety: NCAA – Nigerian Civil Aviation Authority FAA – Federal Aviation Administration ICAO – International Civil Aviation Organization EASA – European Union Aviation Safety Agency NTSB – National Transportation Safety Board May the souls of the departed rest in peace. This must be a wake-up call to all agencies responsible for our safety.
