Today we're publishing the first tranche of action plans provided by previously non-compliant hospitals in order to comply with the CMA’s Private Healthcare Market Investigation Order 2014. This is the beginning of a programme of enforcement action against non-complaint hospitals, which will continue until all hospital sites fulfil their obligations under the Order.   It’s positive to see that these hospitals who we contacted about meeting their requirements under the Private Healthcare Markets Order (2014) have provided action plans for getting their data published on to the Private Hospital Information Network (PHIN) website. Approximately 35,000 prospective patients use this information on the PHIN website every month to make informed decisions about choosing the best possible private healthcare for themselves and their families – including the quality of treatment and fees – so it’s vital that they can access it for every private hospital.   The CMA is committed to seeing full compliance across the sector, and PHIN’s starting point is always to help hospitals or consultants follow the rules without the need for enforcement. However, the sector should be in no doubt that the CMA will continue with our enforcement where legal obligations are not being met.

Pharmacy First Reflections You may have picked up the recent analysis of England’s Pharmacy First service released by the CCA which describes a “patchwork quilt” of engagement with wide variance across ICSs. This prompted the HSJ to headline its article a “Postcode lottery” when receiving the service. This variance is as wide as some pharmacies delivering seven times the level of Pharmacy First consultations and this helps demonstrate the potential health benefits that can be gained for both patients and the NHS infrastructure. The question that this analysis raises with me is, “What is the root cause driving the variation?” My experience from Scotland would suggest it lies in the engagement of other health professionals who may feel no real benefit from getting involved. This is what happened when the Scottish Chronic Medication Service was introduced. Further differences between the two Pharmacy First services is that in Scotland it recognises advice, referral and treatment options, that are available to everyone registered with a Scottish GP on a self-presenting basis for any common clinical condition. This is supplemented by supporting PGDs. Recent data published by Public Health Scotland shows that the Scottish approach has meant the service has been accessed by 1 in 3 of the population and is assisting in addressing health inequalities as the citizens accessing Pharmacy First are assessed as being in the two highest deprivation quintiles. Since its launch the number of consultations has more than doubled and there has been no government publicity campaign. Civil servant colleagues advised that large scale NHS service campaigns have little effect and the best way to encourage service participation is a positive experience in the pharmacy itself when the patient presents. You asked me to identify the major themes for the forthcoming 10-year health plan. These include the following:; o Re-engage staff and re-empower patients. Despite all the challenges and low morale, NHS staff are profoundly passionate and motivated to raise the quality of care for patients. Their talents must be harnessed to make positive change. The best change empowers patients to take as much control of their care as possible. o Lock in the shift of care closer to home by hardwiring financial flows. General practice, mental health and community services will need to expand and adapt to the needs of those with long-term conditions whose prevalence is growing rapidly as the population age. Financial flows must lock-in this change irreversibly or it will not happen. o Simplify and innovate care delivery for a neighbourhood NHS. The best way to work as a team is to work in a team: we need to embrace new multidisciplinary INDEPENDENT INVESTIGATION OF THE NATIONAL HEALTH SERVICE IN ENGLAND | 13 models of care that bring together primary, community and mental health services. o Drive productivity in hospitals. Acute care providers will need to bring down waiting lists by radically improving their productivity. That means fixing flow through better operational management, capital investment in modern buildings and equipment, and re-engaging and empowering staff. o Tilt towards technology. There must be a major tilt towards technology to unlock productivity. In particular, the hundreds of thousands of NHS staff working outside hospitals urgently need the benefits of digital systems. There is enormous potential in AI to transform care and for life sciences breakthroughs to create new treatments. o Contribute to the nation’s prosperity. With the NHS budget at £165 billion this year, the health service’s productivity is vital for national prosperity. Moreover, the NHS must rebuild its capacity to get more people off waiting lists and back into work. At the same time, it should better support British biopharmaceutical companies. o Reform to make the structure deliver. While a top-down reorganisation of NHS England and Integrated Care Boards is neither necessary nor desirable, there is more work to be done to clarify roles and accountabilities, ensure the right balance of management resources in different parts of the structure, and strengthen key processes such as capital approvals. Change will only be successful if the NHS can recover its capacity to deliver plans and strategies as well as to make them. * * *

The NHS Constitution states that patients have the right to access medicines recommended by NICE if clinically appropriate. However, there is variation in access to these medicines across ICSs. There are various reasons for this, including the differing financial contexts of ICBs, which particularly impacts medicines that are expensive and service heavy. Variation in the RAG status of medicines appointed by local formularies can also cause inequity of access to medicines across health and care settings within systems.
