Another interesting episode of Beyond the Blueprint - Health Series focused on hospital at home. One thing that surprised me was that in the home model patients can often tap a button and speak to their nurse right away, whereas in the hospital, there is always a delay between pressing the nurse call button and talking to someone. Back in November 2023, KLAS hosted the third Patient & Consumer Innovation (PCI) Summit in Salt Lake City, Utah. Ten years ago I founded Ensemble Health Partners for two purposes – helping hospitals and being a great place for people to work and grow. My passion for continuing to improve healthcare and belief in loving what you do, providing excellent service and putting people first is at the core of what I do every day, and the foundation I built Ensemble upon. Since 2014, we’ve grown to support hundreds of hospitals and health systems across the U.S. with holistic revenue cycle management. We take pride in helping these providers get paid accurately and on time for the important care they deliver in their communities, and we believe our mission is more critical now than ever. Unfortunately, healthcare organizations are under immense financial strain, struggling to keep up with rising costs, diminishing reimbursement and increasing administrative complexity. According to the American Hospital Association, hospital expenses increased nearly 18% between 2019 and 2022, but their reimbursement from Medicare during that time increased less than 8%. At the same time, health insurance companies are seeing record profits and are investing heavily in advanced technology to scrutinize, delay and deny payments to healthcare organizations. With limited capital, providers are struggling to balance necessary investments in patient care with investments in revenue cycle technology and staffing to keep pace with payer-imposed complexity. As a result, more hospitals closed in 2023 than opened and nearly half of people surveyed said they’ve delayed or foregone care due to rising cost. Hundreds of rural hospitals are at risk of closing, putting millions of Americans at risk of losing local access to an emergency room. We believe that healthcare providers, patients and our communities deserve better and we believe partnerships, collaboration and innovation will play a key role in making the healthcare ecosystem more equitable for all. At Ensemble, we will continue to advocate on behalf of providers and patients across the country for adequate payments and a better healthcare payment system. We will continue working with payers and lawmakers to implement common sense policy changes, reduce unnecessary administrative complexity and make care more accessible and cost-effective for all. We will continue to build and deploy innovative solutions to streamline data exchange between payers and providers and make it easier for healthcare organizations to get paid quickly and accurately for the important care they deliver in our communities. Patient pathways are not static - they are "living artifacts" that evolve as part of their healthcare environment and the organizations involved. Just like patients, patient pathways have a lifecycle. Our book, "Patientenpfade in der integrierten Versorgung", delves deep into the development, implementation, and evaluation of these pathways, offering a scientifically validated and practically tested method. 📖 𝗧𝗵𝗲 𝗣𝗮𝘁𝗵𝘄𝗮𝘆 𝗟𝗶𝗳𝗲𝗰𝘆𝗰𝗹𝗲 𝗮𝘁 𝗮 𝗚𝗹𝗮𝗻𝗰𝗲: 1️⃣ 𝗜𝗻𝗶𝘁𝗶𝗮𝗹 𝗣𝗹𝗮𝗻𝗻𝗶𝗻𝗴 𝗮𝗻𝗱 𝗗𝗲𝘃𝗲𝗹𝗼𝗽𝗺𝗲𝗻𝘁: From needs assessments to iterative design, developing a patient pathway involves five key phases: - Initiating and planning the project. - Analyzing current practices and evidence. - Preparing and structuring the pathway design. - Iteratively creating and refining the pathway. - Communicating and distributing it within the network. 2️⃣ 𝗜𝗺𝗽𝗹𝗲𝗺𝗲𝗻𝘁𝗮𝘁𝗶𝗼𝗻, 𝗨𝘀𝗮𝗴𝗲, 𝗖𝗼𝗻𝘁𝗶𝗻𝘂𝗼𝘂𝘀 𝗠𝗼𝗻𝗶𝘁𝗼𝗿𝗶𝗻𝗴 𝗮𝗻𝗱 𝗖𝗼𝗻𝘁𝗿𝗼𝗹𝗹𝗶𝗻𝗴: Once developed, pathways must be integrated into the care network, adapting existing processes to match pathway specifications. This can range from basic organizational implementation to full digital integration with care network management systems. 3️⃣ 𝗘𝘃𝗮𝗹𝘂𝗮𝘁𝗶𝗼𝗻: Patient pathways must remain dynamic and strive for continuous improvement. Changes like updated medical guidelines or shifts in network composition demand regular evaluation. By continuously measuring pathway effectiveness and adapting pathways, networks can address gaps and enhance care delivery. 4️⃣ 𝗣𝗮𝘁𝗵𝘄𝗮𝘆 𝗔𝗱𝗷𝘂𝘀𝘁𝗺𝗲𝗻𝘁: This phase involves refining processes, testing updates, and aligning the pathway with current best practices to maintain quality and efficiency and to adapt to changing environments. Regular adjustments help address evolving patient and organizational needs. Sometimes, pathways need to be "retired" - when care models change, evidence no longer supports the pathway, or there is no positive impact. 🎁 𝗧𝗮𝗸𝗲𝗮𝘄𝗮𝘆: By following this lifecycle, healthcare organizations can maximize the impact of patient pathways - improving care coordination, resource utilization, and patient outcomes.
