Case Study: PCI for LAD Occlusion in a 72-Year-Old Male performed in Innvolution Cathlab. A 72-year-old male presented with an Anterior Wall Myocardial Infarction (AWMI) and underwent thrombolysis. Despite this treatment, the patient continued to experience angina. The following day, angiography revealed a 100% occlusion in the Left Anterior Descending (LAD) artery, prompting a plan for Percutaneous Transluminal Coronary Angioplasty (PTCA) to the LAD. During pre-dilation, the balloon did not expand due to an eccentric calcium nodule, so a cutting balloon was used instead. Post-dilation angiography showed dissection, and the patient’s blood pressure dropped. The doctor promptly deployed a stent, followed by post-dilation with the balloon, successfully sealing the dissection. The final result was excellent, and the patient remained stable. 🌟 Assessing Coronary Artery Disease Diagnosis with Angio FFR by Medtronic 🌟 An advanced non-invasive technology! 📌Computational Fluid Dynamics: Calculates blood flow and pressure from angiographic images, providing FFR values without the need for pressure wires or hyperemic agents. 📌Optimal Angiograms: Requires high-quality angiographic images for accurate assessments. Ensuring clear visualization and proper opacification of the coronary arteries is essential. 📌Precise Angles: Angio FFR needs 2 to 3 specific projections to capture the coronary tree effectively. Correct angles are critical for accurate 3D reconstruction and reliable FFR values. Does NOT require a wire in the coronary artery. Does NOT require hyperemic agents. Keep innovating! #Cardiology #MedicalInnovation #AngioFFR #Medtronic #CathLab #HealthcareTech #FFR #CardiovascularHealth

Yesterday’s #ClinicalChallenge question was: True or false? In patients with severe aortic stenosis, long-term mortality rates are higher with transcatheter aortic valve implantation (TAVI) compared with surgical aortic valve replacement (SAVR). The answer is: False. In a recent trial in 280 patients with severe aortic stenosis who were randomly assigned to receive a bioprosthetic valve via transcatheter implantation or surgical replacement, mortality rates at 10 years were similar between the two groups. Among patients undergoing mitral valve repair, the addition of tricuspid annuloplasty is associated with a higher incidence of permanent pacemaker implantation, and subsequently linked with reduced long-term survival and higher rates of heart failure hospitalization and infective endocarditis, an analysis of patient data shows. ℹ️ The WATCHMAN™ device, a quarter-sized implant, offers an alternative to blood thinners for patients with atrial fibrillation (AFib). In AF patients, the left atrial appendage (LAA) is a common site for thrombus formation, leading to thromboemboli. The WATCHMAN device serves as a crucial solution in preventing potential complications. 🌟 For AF patients requiring anticoagulation but unable to undergo long-term treatment, the percutaneous LAA occlusion (LAAO) procedure with the WATCHMAN device emerges as an innovative alternative. This procedure ensures effective management based on stroke risk and contraindications to traditional anticoagulants. ℹ️ #WATCHMANdevice #AFib #LAAocclusion #innovativetreatment #cardiology #healthcare #USCE #Match #ECFMG #Clinicalrotations #AAMC #Observership

When it comes to treating severe aortic stenosis, the choice between Balloon-Expandable (BEV) and Self-Expanding (SEV) valves can make all the difference. As an Associate Director in TAVI and Structural Heart Disease, I’ve seen firsthand how crucial this decision is for our patients. Each valve type has its unique strengths, and understanding these can lead to better outcomes and quicker recoveries. Here’s a quick breakdown: Balloon-Expandable Valve (BEV): - How it Works: Delivered on a collapsed catheter and expanded with a balloon. - Best For: Patients with simpler valve anatomy. Pros: - Lower risk of paravalvular leaks - Reduced pacemaker dependency - Strong clinical outcomes Cons: - Slower expansion may not suit all patients - Requires precise placement adjustments Self-Expanding Valve (SEV): - How it Works: Delivered in a compressed form and automatically expands when positioned. - Best For: Patients with complex anatomical structures. Pros: - Adapts well to challenging anatomy - Potentially shorter procedure times - Lower profile delivery system Cons: - Higher risk of paravalvular leaks - Increased pacemaker dependency Why This Matters: The choice between BEV and SEV significantly impacts not just procedure time but also recovery and long-term success. I’ve learned that discussing these options thoroughly with patients helps them feel empowered in their care journey. #TAVR #AorticStenosis #BEV #SEV #Cardiology #PatientCare #HealthcareInnovation
