JACEP Open: Noninvasive respiratory support in the emergency department: Controversies and state-of-the-art recommendations This article discusses the use of noninvasive respiratory support modalities, such as Continuous Positive Airway Pressure (CPAP) and Noninvasive Positive Pressure Ventilation (NIPPV), in emergency settings. It emphasizes starting CPAP/EPAP at moderate levels (8–10 cm H₂O) and adjusting based on patient tolerance, while for Nasal High Flow (NHF), initiating at high flow rates to maximize end-expiratory lung volume and then weaning according to the patient's work of breathing is recommended. As a Resiratory Therapist for nearly two decades, I’ve witnessed firsthand the critical role we ERS play in helping patients breathe easier, live fuller lives, and, in many cases, survive challenging medical conditions. We are the frontline advocates for lung health, and our expertise shapes patient outcomes in profound ways. In my nearly 20 years in this field, I’ve learned that Respiratory Therapy is more than a job; it’s a calling. It requires patience, resilience, and a commitment to lifelong learning. Learn the landmarks!!! If you memorize and know what your looking at you will easily understand if your blade is deep or shallow adjust accordingly while observing until your landmarks come into view. When using a Mac blade swipe the tongue to the left. I’ve seen many insert the Mac blade similar to a miller and most the intubation every time. Practice makes perfect
