"A three-day-old female newborn was delivered by cesarean section with a gestational age of 40 weeks and a birth weight of 2,650 g. She was admitted to the newborn intensive care unit with the diagnosis of intestinal obstruction. A periumbilical incision was performed using an Alexis (Applied Medical Resources Corporation, Rancho Santa Margarita, CA) wound protector/retractor. An apple peel intestinal atresia (IIIb) was observed. The dilated bowel was 20 cm long from the Treitz ligament to the atretic site, while the length of the distal segment was 40 cm long. The size discrepancy between proximal and distal bowel was 8:1." An excellent example of how Alexis device helps in daily physicians practice.

Previously manipulated lesions are a challenge to resect. Adolfo Parra-Blanco schedules an entire day every month to tackle these difficult polyps. Though very experienced at ESD, he prefers a multi-modal approach (EMR + Endorotor +/- APC) that he recently showed to eradicate 93% of the previously manipulated polyps within three sessions. (Rehman et al. 2024). Whereas, conventional techniques (EMR +/- APC) show a 53% tissue persistence rate for similar lesions (Kim et al. 2015). ✨ Promising results on the use of triamcinolone in circumferential esophageal ESD ✨ Spectacular results have been published this month in Gut (Zhan K, Wang P, Liu W, et al. Gut 2024;73:1780–1784) regarding the use of triamcinolone on days 0(intraprocedural and postprocedure), 5 and 12 following circumferential esophageal ESD. Although the study involved only 9 patients, they reported just 1 stricture, which required only one dilation! This is a truly promising advancement, offering significant potential to improve the quality of life for patients after cESD.
